综合改善睡眠时间措施联合氨氯地平对老年原发性高血压伴睡眠障碍的临床效果观察

摘　　要：目的分析综合改善睡眠时间措施联合氨氯地平对老年原发性高血压伴睡眠障碍患者的临床效果。方法100例老年原发性高血压伴睡眠障碍患者,随机分为观察组和对照组,各50例。对照组给予口服苯磺酸氨氯地平片治疗,观察组在对照组的基础上给予综合改善睡眠时间措施。比较两组患者的临床疗效、血压改善情况及治疗前后匹兹堡睡眠质量指数。结果观察组患者的治疗总有效率88.0%高于对照组的60.0%,差异有统计学意义(P<0.05)。观察组患者的收缩压(119.2±4.8)mm Hg(1 mm Hg=0.133 kPa)、舒张压(75.4±4.4)mm Hg均明显低于对照组的(138.5±5.6)、(81.3±3.9)mm Hg,差异有统计学意义(P<0.05)。治疗前,两组患者的匹兹堡睡眠质量指数比较,差异无统计学意义(P>0.05);治疗后,观察组患者的匹兹堡睡眠质量指数(8.54±2.34)分低于对照组的(17.25±3.17)分,差异有统计学意义(P<0.05)。结论使用综合改善睡眠时间措施联合氨氯地平治疗老年原发性高血压伴睡眠障碍患者效果明显,可有效控制患者的血压,改善睡眠质量,值得广泛推荐使用。Objective To analyze the clinical effect of comprehensive sleep improvement measures combined with amlodipine in the treatment of essential hypertension with sleep disorders in the elderly.Methods A total of 100 elderly patients with essential hypertension and sleep disorders were randomly divided into observation group and control group,with 50 cases in each group.The control group was treated with oral amlodipine besylate tablets,and the observation group was treated with comprehensive sleep improvement measures on the basis of the control group.The clinical efficacy,blood pressure improvement and Pittsburgh sleep quality index before and after treatment were compared between the two groups.Results The total effective rate 88.0%in the observation group was higher than 60.0%in the control group,and the difference was statistically significant(P<0.05).The systolic blood pressure(119.2±4.8)mm Hg(1 mm Hg=0.133 kPa)and diastolic blood pressure(75.4±4.4)mm Hg of the observation group were significantly lower than(138.5±5.6)and(81.3±3.9)mm Hg of the control group,and the differences were statistically significant(P<0.05).Before treatment,there was no statistically significant difference in Pittsburgh sleep quality index score between the two groups(P>0.05).After treatment,the Pittsburgh sleep quality index(8.54±2.34)points of the observation group was lower than(17.25±3.17)points of the control group,and the difference was statistically significant(P<0.05).Conclusion Comprehensive sleep improvement measures combined with amlodipine has obvious effect in the treatment of essential hypertension with sleep disorders in the elderly,and can effectively control the blood pressure and improve the sleep quality of patients,which is widely recommended for use.
