摘  要:目的研究核糖核酸(RNA)恒温扩增实时检测技术和荧光定量聚合酶链式反应(PCR)技术联合应用于痰涂片阴性肺结核患者诊断中的临床价值。方法80例痰涂片阴性肺结核患者,在治疗前分别采用RNA恒温扩增实时检测技术、荧光定量PCR技术、两项技术联合方式对肺泡灌洗液标本进行检测。比较三种检测方法的阳性率、操作时间、确诊时间、住院时间、满意度。结果联合检测阳性率96.25%高于RNA恒温扩增实时检测技术的75.00%、荧光定量PCR技术的78.75%,差异有统计学意义(P<0.05)。RNA恒温扩增实时检测技术与荧光定量PCR技术检测的阳性率比较,差异无统计学意义(P>0.05)。联合检测操作时间(42.19±8.10)min长于RNA恒温扩增实时检测技术的(28.16±5.08)min、荧光定量PCR技术的(26.73±6.95)min,确诊时间(1.68±0.35)h、住院时间(10.27±2.55)d短于RNA恒温扩增实时检测技术的(4.24±0.46)h、(19.76±2.51)d及荧光定量PCR技术的(4.69±0.72)h、(18.20±2.68)d,差异有统计学意义(P<0.05)。RNA恒温扩增实时检测技术与荧光定量PCR技术的操作时间、确诊时间、住院时间比较,差异无统计学意义(P>0.05)。联合检测的总满意度为96.25%,高于RNA恒温扩增实时检测技术的78.75%与荧光定量PCR技术的82.50%,差异有统计学意义(P<0.05)。RNA恒温扩增实时检测技术与荧光定量PCR技术的总满意度比较,差异无统计学意义(P>0.05)。结论RNA恒温扩增实时检测技术和荧光定量PCR技术联合应用于痰涂片阴性肺结核诊断,可缩短确诊时间,提高患者满意度,值得临床应用。Objective To study the clinical value of real-time isothermal ribonucleic acid(RNA)amplification combined with fluorescence quantitative polymerase chain reaction(PCR)in sputum smearnegative pulmonary tuberculosis patients.Methods In 80 patients with sputum smear-negative pulmonary tuberculosis,the alveolar lavage fluid specimens were tested by single and combination of real-time isothermal RNA amplification and fluorescence quantitative PCR before treatment.The positive rate,operation time,time to diagnosis,hospitalization time,and overall satisfaction of the three test methods were compared.Results The positive rate of combined detection was 96.25%,which was higher than 75.00% of real-time isothermal RNA amplification and 78.75% of fluorescence quantitative PCR,and the differences were statistically significant(P<0.05).There was no statistically significant difference in the positive rates of real-time isothermal RNA amplification combined with fluorescence quantitative PCR(P>0.05).The operation time(42.19±8.10)min of combined detection was longer than(28.16±5.08)min of real-time isothermal RNA amplification and(26.73±6.95)min of fluorescence quantitative PCR;the time to diagnosis(1.68±0.35)h and hospitalization time(10.27±2.55)d of combined detection were shorter than(4.24±0.46)h,(19.76±2.51)d of real-time isothermal RNA amplification and(4.69±0.72)h,(18.20±2.68)d of fluorescence quantitative PCR;all the differences were statistically significant(P<0.05).There was no statistically significant difference in the operation time,time to diagnosis,and hospitalization time between real-time isothermal RNA amplification and fluorescence quantitative PCR(P>0.05).The overall satisfaction with the combined test was 96.25%,which was higher than 78.75% for real-time isothermal RNA amplification and 82.50% for fluorescence quantitative PCR,and the differences were statistically significant(P<0.05).There was no statistically significant difference in the overall patient satisfaction between real-time isothermal RNA
