糖皮质激素联合免疫球蛋白治疗晚期妊娠合并血小板减少症分娩产妇的效果观察

摘　　要：目的探讨糖皮质激素联合免疫球蛋白治疗晚期妊娠合并血小板减少症分娩产妇的效果。方法100例晚期妊娠合并血小板减少症分娩产妇,按照治疗方法的不同分为联合治疗组与糖皮质激素组,每组50例。联合治疗组产妇采用糖皮质激素联合免疫球蛋白进行治疗,糖皮质激素组产妇则采用糖皮质激素治疗。比较两组血小板计数恢复时间、临床疗效、不良反应发生率、妊娠结局。结果联合治疗组血小板计数恢复时间(8.43±1.32)d短于糖皮质激素组的(11.82±3.24)d,差异有统计学意义(P<0.05)。联合治疗组治疗总有效率为96.00%,高于糖皮质激素组的74.00%,差异有统计学意义(P<0.05)。联合治疗组不良反应发生率为2.00%,低于糖皮质激素组的20.00%,差异有统计学意义(P<0.05)。联合治疗组不良妊娠结局发生率为18.00%,低于糖皮质激素组的36.00%,差异有统计学意义(P<0.05)。结论对晚期妊娠合并血小板减少症分娩产妇施以糖皮质激素治疗联合免疫球蛋白治疗,可有效改善血小板计数情况和妊娠结局,减少不良反应,降低早产和流产率。Objective To discuss the effect of glucocorticoid combined with immunoglobulin in the treatment of puerperae with thrombocytopenia in late pregnancy.Methods A total of 100 parturients with thrombocytopenia in late pregnancy were divided into combined treatment group and glucocorticoid group according to different treatment methods,with 50 cases in each group.The parturients in the combined treatment group were treated with glucocorticoid combined with immunoglobulin,while the parturients in the glucocorticoid group were treated with glucocorticoid.The recovery time of platelet count,clinical efficacy,incidence of adverse reactions,and pregnancy outcomes were compared between the two groups.Results The recovery time of platelet count in the combined treatment group was(8.43±1.32)d,which was shorter than(11.82±3.24)d in the glucocorticoid group,and the difference was statistically significant(P<0.05).The total effective rate in the combined therapy group was 96.00%,which was higher than 74.00% in the glucocorticoid group,and the difference was statistically significant(P<0.05).The incidence of adverse reactions in the combined treatment group was 2.00%,which was lower than 20.00% in the glucocorticoid group,and the difference was statistically significant(P<0.05).The incidence of adverse pregnancy outcomes in the combined treatment group was 18.00%,which was lower than 36.00% in the glucocorticoid group,and the difference was statistically significant(P<0.05).Conclusion For puerperae with thrombocytopenia in late pregnancy,glucocorticoid combined with immunoglobulin can effectively improve the platelet count and pregnancy outcome,reduce adverse reactions,and reduce the rate of premature delivery and abortion.
