摘 要:目的了解及分析2018—2020年江阴地区0-14岁10940例急性呼吸道感染住院儿童8种常见呼吸道病原体检测结果及其流行学特点。方法选择2018年1月—2020年12月本院收治的10940例0-14岁急性呼吸道感染住院患儿作为研究对象,入院时同时采集鼻咽拭子及血标本,运用直接免疫荧光法(DFA)及间接免疫荧光法对呼吸道合胞病毒(RSV)、腺病毒(ADV)、甲型流感病毒(FluA)、乙型流感病毒(FluB)、副流感病毒(PIV-1、2、3)及肺炎支原体(MP)进行检测,以了解及分析患儿呼吸道病原体流行特点。结果在所调查的样本中4795例儿童检出至少1种呼吸道病原体感染,感染率为43.83%(4795/10940);其中单一病原体占92.70%(4445/4795),混合感染占7.30%(350/4795);其前3位的呼吸道病原体分别为MP 51.64%(2746/4795),RSV 24.34%(1167/4795),PIV-38.70%(417/4795);男性患儿总体感染率为40.45%(2475/6119),女性为48.10%(2319/4821),两者差异有统计学意义(P<0.05);在不同年龄段儿童中,MP、RSV、FluB、PIV-1、PIV-2、PIV-3、ADV感染率差异具有统计学意义(P<0.05),其中新生儿期及婴儿期RSV阳性率最高,在幼儿期MP及RSV阳性率最高,学龄前期、学龄期及青春期MP阳性率最高;不同季节中,该地区呼吸道病原体感染率差异具有统计学意义(P<0.05),主要表现在MP,RSV及PIV-3,MP感染率夏秋高于冬春季节,RSV冬季感染率较高,PIV-3秋季感染率较高。结论江阴地区2018—2020年住院患儿急性呼吸道感染病原体流行病学分布随性别、年龄、季节而不同,其中以MP、RSV及PIV-3为主,早期的呼吸道病原体检测,有利于儿童相关呼吸疾病及早预防及诊治。Objective To understand and analyze the detection results and epidemiological characteristics of 8 common respiratory pathogens in 10940 hospitalized children who suffered from acute respiratory tract infections aged 0-14 years in the Jiangyin area from 2018 to 2020.Methods 10940 hospitalized children suffered from acute respiratory tract infections aged 0-14 years in Jiangyin people’s hospital from January 2018 to December 2020 were selected as study subjects.Nasopharyngeal swabs and blood samples were collected at the same time.Respiratory syncytial virus(RSV),adenovirus(ADV),influenza A virus(FluA),influenza B virus(Flub),Parainfluenza virus(PIV-1,2,3),and Mycoplasma pneumoniae(MP)were detected by direct immunofluorescence(DFA)and indirect immunofluorescence to understand and analyze the epidemic characteristics of respiratory pathogens in children.Results In the investigated samples,4795 children were infected with at least one respiratory pathogen,and the infection rate was 43.83%(4795/10940),of which single pathogen infection accounted for 92.70%(4445/4795),and mixed infection accounted for 7.30%(350/4795).The top three respiratory pathogens were MP(51.64%,2746/4795),RSV(24.34%,1167/4795)and PIV-3(8.70%,417/4795).The total infection rate of male children was 40.45%(2475/6119),and that of female children was 48.10%(2319/4821),and there was a significant difference between male and female children(P<0.05).There were significant differences in the infection rates of MP,RSV,Flub,PIV-1,PIV-2,PIV-3,and ADV among children of different ages(P<0.05).During the neonatal period and infant period,RSV positive rate was the highest;during early childhood,MP and RSV positive rates were the highest;during the preschool age,school age and adolescence,MP positive rate was the highest.In different seasons,the difference in respiratory pathogen infection rate in this area was statistically significant(P<0.05),and the differences mainly manifested in MP,RSV,and PIV-3.The infection rate of MP in summer and autumn was higher th
