中药局部冷敷对下肢深静脉血栓的影响

摘　　要：目的探究中药局部冷敷对下肢深静脉血栓的影响。方法选取西安中医脑病医院2021年1月至12月收治的100例下肢深静脉血栓患者,分为对照组和观察组,各50例。对照组采用常规西医治疗,观察组在对照组的基础上联合使用中药局部冷敷治疗,比较两组临床疗效和凝血酶原时间(prothrombin time,PT)、活化部分凝血活酶时间(activated partial thromboplastin time,APTT)、纤维蛋白原(fibrinogen,FIB)、D-二聚体(D-dimer,D-D)等各凝血指标,纤溶酶原(plasminogen,Plg)活性、组织型纤溶酶原激活物(tissue plasminogen activator,tPA-A)活性、纤溶酶原激活物抑制物-1(plasminogen activator inhibitor-1,PAI-1)等各纤溶指标。结果观察组总有效率高于对照组(P<0.05);治疗前,两组各凝血指标差异无统计学意义(P>0.05),治疗后,观察组APTT、PT水平高于治疗前与对照组,FIB、D-D水平低于治疗前与对照组,差异有统计学意义(P<0.05);治疗前,两组各纤溶指标差异无统计学意义(P>0.05),治疗后,观察组PIg、tPA-A含量高于治疗前与对照组,PAI-1含量低于治疗前与对照组,差异有统计学意义(P<0.05)。结论中药局部冷敷治疗下肢深静脉血栓疗效确切,可改善血液高凝状态和纤溶指标。Objective To explore the effect of local cold compress of traditional Chinese medicine on deep venous thrombosis of lower extremity.Methods 100 patients with deep venous thrombosis of lower extremities admitted to Xi’an Traditional Chinese Medicine Encephalopathy Hospital from January to December 2021 were divided into control group and observation group,with 50 cases in each group.The control group was treated with conventional western medicine,and the observation group was treated with local cold compress of traditional Chinese medicine on the basis of the control group.The clinical efficacy and coagulation indexes[prothrombin time(PT),activated partial thromboplastin time(APTT),fibrinogen(FIB),D-dimer(D-D)],and various fibrinolytic indexes[plasminogen(Plg)activity,tissue plasminogen activator(tPA-A)activity,plasminogen activator inhibitor-1(plasminogen activator inhibitor-1,PAI-1)]of the two groups were compared.Results The total effective rate in the observation group was higher than that in the control group(P<0.05).Before treatment,there was no significant difference in coagulation indexes between the two groups(P>0.05);after treatment,the levels of APTT and PT in the observation were higher than those before treatment and those in the control group,and the levels of FIB and D-D in the observation group were lower than those before treatment and the control group,and the difference was statistically significant(P<0.05).Before treatment,there was no significant difference in fibrinolysis indexes between the two groups(P>0.05);after treatment,the contents of PIg and tPA-A in the observation group were higher than those before treatment and those in the control group,and the contents of PAI-1were lower than those before treatment and those in the control group,and the difference was statistically significant(P<0.05).Conclusion Local cold compress of traditional Chinese medicine is effective in treating deep vein thrombosis of lower extremities,and can improve blood hypercoagulability and fibrinolysis indexes
