摘　　要：目的:探讨精神专科医院失眠症患者第二类精神药品依赖的相关影响因素。方法:以2021年7~12月在该院门诊就诊的失眠症患者为研究对象,经培训的医务人员使用自编的第二类精神药品使用情况调查表对符合标准的患者进行调查,对比病例组(诊断为第二类精神药品依赖患者)与对照组(未依赖患者)资料,采用Logistic回归模型分析第二类精神药品依赖的影响因素。结果:获有效调查问卷525份,筛选出第二类精神药品依赖患者51例,占9.71%。单因素分析显示,两组患者年龄、病前性格、与家人关系、经历重大事件、酒精依赖、二类精神用药时长、服药依从性、随诊等特征的分布差异有统计学意义(P<0.05);Logistic回归模型分析结果显示,与家人关系和睦、未经历重大事件和无酒精依赖是失眠症患者第二类精神药品依赖的保护因素,服药依从性差、不随诊是危险因素。结论:服用第二类精神药品发生依赖的风险较高,应加强合理用药知识宣教,对于经历过重大事件和酒精依赖的患者应谨慎开具该类药品,建议对正在服用或将开具该类药物的门诊患者加强管理。Objective:To explore the related influence factors of second class psychotropic drug dependence in insomnia patients in psychiatric hospital. Methods:Insomnia patients in the outpatient department of the hospital from July 2021 to December 2021 were taken as the research object. The trained medical staff used the self-made questionnaire on the use of second class psychotropic drugs to investigate the patients who met the standards. The data of the case group(patients with second class psychotropic drug dependence) and the control group(non dependent patients) were compared. The influencing factors of the second type of psychotropic drug dependence were analyzed by logistic regression. Results:A total of 525 valid questionnaires were obtained, and 51 patients with second class psychotropic drug dependence were selected, accounting for 9.71%. Univariate analysis showed that there were statistically significant difference in age, personality before illness, relationship with family, experience of major events, alcohol dependence, duration of second class psychotropic drugs, compliance and follow-up between two groups(P<0.05);Logistic regression analysis showed that harmonious relationship with family, no major events and no alcohol dependence were protective factors, poor compliance and no follow-up were risk factors.Conclusion:The risk of dependence on second class psychotropic drugs is high. We should strengthen the publicity and education of rational drug use. Patients who have experienced major events and alcohol dependence should be cautious about issuing such drugs. It is suggested to strengthen the management of outpatients who are taking or intend to issue such drugs.
