摘　　要：临床药师参与1例念珠菌血流感染并肺曲霉菌重症感染患者的治疗过程,利用药学思维探索病因发现药品不良反应,协助医生共同制定药物治疗方案,深入查询循证资料,客观评估病情,判断定植菌,避免抗菌药物的过度使用,监测药物不良反应,进行用药教育,全方位提供药学服务。临床药师的建议基本被采纳,抗感染治疗有效,患者对治疗效果和药学服务满意。临床药师参与治疗团队,充分发挥药学专业优势,保证了药物治疗的安全、有效、经济。Our clinical pharmacist have heavily involved in a patient rescue who suffered candidemia and severe pulmonary aspergillus infections. The contibution of the pharmacist includes, but not limited to, exploring the causes of disease from the angle of medical science;monitoring ADR;developing drug treatment regimes with Doctors;seeking help from "Evidence-based medicine" data;providing advice on patient’s condition;finding bacteriopexia and avoiding overused antibiotics;providing education of dosage knowledge;and so on. And most encouragingly, the suggestion from clinical pharmacist almost had been adopted, which ensures the effective anti-infection and patient’s satisfaction. It is obviously, the involvment of clinical pharmacist in medical treatment can offer much help on safe, effective and economical drug therapy.
