滋阴泻火方治疗单纯性乳房早发育患儿临床观察

摘　　要：【目的】观察滋阴泻火方治疗阴虚火旺型单纯性乳房早发育女童的临床疗效。【方法】将76例阴虚火旺型单纯性乳房早发育患儿随机分为治疗组和对照组,每组各38例。对照组单纯给予饮食生活指导,治疗组在对照组饮食生活指导的基础上给予口服中药方剂滋阴泻火方治疗,疗程为4周。观察2组患儿治疗前后乳房发育分期评分及中医证候评分的变化情况,评价2组患者的总体临床疗效、中医证候疗效及安全性。【结果】(1)脱落情况:研究过程中,治疗组脱落6例,对照组脱落2例;最终共纳入68例患儿,其中治疗组32例,对照组36例。(2)总体临床疗效情况:治疗4周后,治疗组的总有效率为78.1%(25/32),对照组为19.4%(7/36),组间比较,治疗组的总体临床疗效明显优于对照组,差异有统计学意义(P<0.05)。(3)中医证候疗效情况:治疗4周后,治疗组的总有效率为78.1%(25/32),对照组为13.9%(5/36),组间比较,治疗组的中医证候疗效明显优于对照组,差异有统计学意义(P<0.05)。(4)乳房发育分期评分及中医证候评分情况:治疗后,治疗组患儿的乳房发育分期评分及中医证候评分均较治疗前明显降低(P<0.05),而对照组均无明显变化(P>0.05);组间比较,治疗组患儿的乳房发育分期评分及中医证候评分均明显低于对照组,差异均有统计学意义(P<0.05)。(5)安全性情况:治疗期间,2组患儿均无明显不良反应发生。【结论】滋阴泻火方治疗女童阴虚火旺型单纯性乳房早发育疗效确切,能有效降低患儿的乳房Tanner分期,改善患儿的中医临床症状。Objective To observe the clinical efficacy of Ziyin Xiehuo Formula in treating young girls with simple premature thelarche(PT)of yin deficiency and fire hyperactivity syndrome type.Methods Seventy-six cases of yin deficiency and fire hyperactivity syndrome type of simple PT were randomly divided into the treatment group and the control group,with 38 cases in each group.The control group was given dietary and living guidance alone,while the treatment group was given oral administration of Ziyin Xiehuo Formula on the basis of treatment for the control group.The treatment for both groups lasted for 4 weeks.The changes in breast development staging scores and traditional Chinese medicine(TCM)syndrome scores in the two groups were observed before and after treatment.After treatment,the overall clinical efficacy,TCM syndrome efficacy,and clinical safety in the two groups were evaluated.Results(1)During the trial,6 cases in the treatment group and 2 cases in the control group fell off,and a total of 68 children eventually completed the trial,including 32 cases in the treatment group and36 cases in the control group.(2)After 4 weeks of treatment,the overall effective rate of the treatment group was78.1%(25/32)and that of the control group was 19.4%(7/36).The intergroup comparison showed that the overall clinical efficacy of the treatment group was significantly superior to that of the control group,and the difference was statistically significant(P<0.05).(3)After 4 weeks of treatment,the overall effective rate for the TCM syndrome efficacy of the treatment group was 78.1%(25/32),and that of the control group was 13.9%(5/36).The intergroup comparison showed that the TCM syndrome efficacy in the treatment group was significantly superior to that in the control group,and the difference was statistically significant(P<0.05).(4)After treatment,the breast development staging scores and TCM syndrome scores of children in the treatment group were significantly decreased compared with those before treatment(P<0.05),while there wa
