基于“从瘀论治”“内外并治”理论运用苍柏止痒方治疗原发性肛周湿疹的临床疗效观察

摘　　要：【目的】探讨基于“从瘀论治”“内外并治”理论,运用自拟苍柏止痒方口服和坐浴治疗原发性肛周湿疹的临床疗效。【方法】将66例原发性肛周湿疹患者随机分为治疗组和对照组,每组各33例。对照组给予温水坐浴和曲咪新乳膏外用治疗,治疗组在对照组的基础上联合苍柏止痒方口服和坐浴治疗,疗程为2周。观察2组患者治疗前后瘙痒程度视觉模拟量表(VAS)评分、湿疹面积及严重程度(EASI)评分、中医证候积分、皮肤病生活质量指数(DLQI)评分的变化情况,评价2组患者的临床疗效及安全性,比较2组患者疗程结束2个月后随访的复发情况。【结果】(1)治疗2周后,治疗组的总有效率为100.0%(33/33),对照组为33.3%(11/33),组间比较,治疗组的临床疗效明显优于对照组,差异有统计学意义(P<0.01)。(2)治疗后,2组患者的瘙痒程度VAS评分、EASI评分、中医证候积分和DLQI评分均较治疗前明显降低(P<0.01),且治疗组的降低幅度均明显优于对照组,差异均有统计学意义(P<0.01)。(3)疗程结束2个月后随访,治疗组的复发率为15.2%(5/33),明显低于对照组的84.8%(28/33),差异有统计学意义(P<0.01)。(4)研究期间,2组患者均未出现不良反应,且各项安全性指标均在正常范围内,具有较高的安全性。【结论】联合自拟苍柏止痒方口服和坐浴治疗原发性肛周湿疹,不仅能较好地改善患者局部症状及体征,显著提高临床疗效,还可有效防止湿疹再发,改善湿疹患者生活质量,其疗效优于对照组单纯温水坐浴和曲咪新乳膏外用治疗。Objective To observe the clinical efficacy of the self-prescribed Cang Bo Itching-relieving Formula for oral use and sitz bath in treating primary perianal eczema based on the theory of“treating from stasis”and“treating with both internal and external therapy”.Methods Sixty-six patients with primary perianal eczema were randomly divided into the treatment group and the control group,with 33 cases in each group.The control group was given sitz bath with warm water and external application of triamcinolone acetonide acetate cream,while the treatment group was treated with self-prescribed Cang Bo Itching-relieving Formula for oral use and sitz bath on the basis of treatment for the control group.The treatment for both groups lasted for 2 weeks.The changes of visual analogue scale(VAS)scores of itching,eczema area and severity index(EASI)scores,traditional Chinese medicine(TCM)syndrome scores,and dermatological life quality index(DLQI)scores in the two groups before and after treatment were observed.Meanwhile,the clinical efficacy and safety of the two groups were evaluated,and the recurrence in the two groups during the follow-up 2 months after the completion of treatment was compared.Results(1)After 2 weeks of treatment,the total effective rate of the treatment group was 100.0%(33/33)and that of the control group was 33.3%(11/33).The intergroup comparison showed that the clinical efficacy of the treatment group was significantly superior to that of the control group,and the difference was statistically significant(P<0.01).(2)After treatment,the itching VAS scores,EASI scores,TCM syndrome scores,and DLQI scores in the two groups were significantly decreased compared with those before treatment(P<0.01),and the decreased range in the treatment group was significantly superior to that in the control group,the difference being statistically significant(P<0.01).(3)During the follow-up 2 months after the completion of treatment,the recurrence rate of the treatment group was 15.2%(5/33),which was significantly lowe
