基于数据挖掘技术探讨针灸治疗卵巢储备功能下降的选穴规律

摘　　要：【目的】基于数据挖掘技术探讨针灸治疗卵巢储备功能下降的选穴规律。【方法】计算机检索中国期刊全文数据库(CNKI)、万方学术期刊全文数据库(Wanfang)、维普中文科技期刊数据库(VIP)、美国医学在线(PubMed)等数据库中近10年来有关针灸治疗卵巢储备功能下降的临床随机对照试验文献,采用SPSS 22.0统计软件对腧穴频次、部位、归经进行描述性分析,运用SPSS Modeler 18.0软件中的Apriori算法对高频腧穴进行关联规则分析并构建关联网络图。【结果】最终纳入32篇文献,涉及53个腧穴,累计使用频次325次:(1)腧穴使用频次最高者为关元、三阴交;(2)归经频次最高者为任脉,其次是足太阳膀胱经、足少阴肾经、足阳明胃经、足太阴脾经;(3)腧穴分布频次最高者为腹部,其次是下肢;(4)核心腧穴处方为“关元、三阴交、足三里、太溪”,核心穴对为“关元—三阴交”。【结论】针灸治疗卵巢储备功能下降以循经、分部和辨证取穴为原则,注重近部取穴与远部取穴相结合,以腹部、下肢为主,从而达到补肾精、调冲任、行气血的目的。Objective To explore the acupoint selection rules of acupuncture and moxibustion in treating decreased ovarian reserve(DOR)based on the data mining technology.Methods The clinical randomized controlled trials of acupuncture and moxibustion in the treatment of DOR in recent 10 years were screened in China National Knowledge Internet(CNKI),Wanfang Data Knowledge Service Platform(Wanfang),China Science and Technology Journal Database(VIP),PubMed and other databases by computor.SPSS 22.0 statistical software was used to conduct descriptive analysis on the frequency,location and meridian of acupoints,and the Apriori algorithm of SPSS Modeler 18.0 was used to analyze the association rules of high-frequency acupoints and construct the association network diagram.Results A total of 32 literatures were finally included,involving 53 acupoints,which were used 325 times in total:(1)the most frequently used acupoints were Guanyuan(RN4)and Sanyinjiao(SP6);(2)the most frequently used meridian tropism was the conception vessel(CV),followed by the foot taiyang bladder meridians,foot shaoyin kidney meridians,foot yangming stomach meridians and foot taiyin spleen meridians;(3)the most frequently distributed acupoints were in the abdomen,followed by the lower limbs;(4)the core acupoint prescriptions were“Guanyuan(RN4),Sanyinjiao(SP6),Zusanli(ST36)and Taixi(KI3)”,and the core acupoint pair was“Guanyuan(RN4)-Sanyinjiao(SP6)”.Conclusion Acupuncture and moxibustion therapy for DOR should be based on the principles of following the meridians,selecting acupoints according to physical division and syndrome differentiation,paying attention to the combination of neighboring point and distant point selection,mainly focus on the abdomen and lower limbs,so as to achieve the purpose of tonifying kidney essence,regulating conception channels and regulating qi and blood.
