手术间隔时间对分期治疗双侧发育性髋关节脱位疗效的影响

摘　　要：目的:探讨手术间隔时间对分期治疗双侧发育性髋关节脱位疗效的影响。方法:回顾性分析2010年1月至2020年1月郑州大学第一附属医院因双侧发育性髋关节脱位分期接受骨盆Salter截骨术联合股骨截骨术患儿(共26例)的临床资料,根据患儿双侧髋关节手术间隔时间将时间间隔≤8个月组设为观察组,时间>8个月组设为对照组。观察对比两组患儿术后疗效及并发症发生率。结果:观察组手术间隔时间平均为7.1个月,对照组平均间隔时间10.3个月。观察组和对照组术前、术后不同时期髋臼指数和中心边缘角相比差异无统计学意义(P>0.05)。观察组术后髋关节功能优良率(92.9%)高于对照组(70.9%)(P=0.030)。两组并发症发生率差异无统计学意义(P>0.05)。结论:采用切开复位、骨盆Salter截骨联合股骨截骨是治疗双侧发育性髋脱位的有效方法,手术时间间隔≤8个月组整体疗效优于间隔>8个月组。Aim:To investigate the effects of surgical interval on efficacy of staging surgery in patients with bilateral developmental dislocation of hip.Methods:Retrospective analysis was performed on 26 children with bilateral developmental hip dislocation,who received pelvic Salter osteotomy combined with femoral osteotomy in the First Affiliated Hospital of Zhengzhou University,from January 2010 to January 2020.According to the length of the interval between bilateral hip operations,the children with an interval≤8 months were assigned to the observation group,and the children in the group with an interval>8 months were assigned to the control group.The postoperative efficacy and incidence of complications were observed and compared.Results:The mean interval was 7.1 months in the observation group and 10.3 months in the control group,there was no statistical significance in the acetabular index or center-edge angle between the observation group and the control group before and after operation(P>0.05).The postoperative rate of good hip joint function in the observation group was significantly higher than that of the control group(92.9%vs 70.9%,P=0.030).The incidence of complications showed no statistical significance between the two groups(P>0.05).Conclusion:Open reduction,pelvic Salter osteotomy combined with femoral osteotomy is an effective method for the treatment of bilateral developmental hip dislocation.The overall efficacy of bilateral operation interval≤8 months group is better than that of bilateral operation interval>8 months group.
