摘　　要：目的比较ICU-VTE评分表与Caprini评估量表对危重症患者发生静脉血栓栓塞症(VTE)的预测价值。方法分别用ICU-VTE评分表与Caprini评估量表对2018年6月-2021年6月重庆医科大学附属第一医院重症医学科收治的464例ICU患者进行评分,72例(15.5%)患者有静脉血栓,根据是否发生静脉血栓分为2组。采用SPSS 21.0、Medcalc软件对2组患者一般资料、2种评估量表的ROC曲线下面积进行分析。结果ICU-VTE评分表最佳临界点为8分时,可筛选出191例(41.2%)的高危患者,而Caprini评估表最佳临界点为7分时,有209例(45%)的高危患者被识别。ICU-VTE评分表的ROC曲线下面积为0.741,高于Caprini评估量表的0.685;但差异无统计学意义(Z=1.560,P=0.119)。结论ICU-VTE评分表在ICU患者静脉血栓风险预测价值方面不低于Caprini评估表,且其评估条目较少,操作简便易行。Objective To compare the predictive value of the ICU-VTE score scale and Caprini assessment scale for venous thromboembolism(VTE)in critically ill patients.Methods A total of 464 ICU patients admitted to the Intensive Care Unit of the First Affiliated Hospital of Chongqing Medical University from June 2018 to June 2021 were evaluated by the ICU-VTE score scale and Caprini assessment scale respectively.Seventy-two patients(15.5%)with venous thrombosis were divided into two groups according to the occurrence of venous thrombosis.SPSS21.0 and Medcalc softwares were used to analyze the general data of patients in two groups and the areas under ROC curve of the two assessment scales.Results The optimal critical point of the ICU-VTE score scale was 8 points,and 191 patients(41.2%)were identified as high-risk patients,while the optimal point of the Caprini assessment scale was 7 points,and 209 patients(45%)were identified as high-risk patients.The area under ROC curve of ICU-VTE score scale was 0.741,which was higher than 0.685 of the Caprini assessment scale.However,there was no statistical difference in the area under ROC curve(Z=1.560,P=0.119).Conclusions The ICU-VTE score scale is no less valuable than the Caprini assessment scale in predicting the risk of VENOUS thrombosis in ICU patients,and it has fewer evaluation items and is easy to operate.
