摘 要:目的探讨非注气法床旁留置空肠营养管在有创人工气道危重患者中的临床应用效果。方法选取2020年4月-2021年4月需留置空肠营养管且带有有创人工气道的患者92例。将2020年4月1日-2020年10月1日的需要留置空肠营养管的50例患者作为对照组,采用常规留置空肠营养管的方法,当空肠营养管置入胃内后,予患者胃内置入300~500 mL空气,以增加胃的蠕动能力;将2020年10月2日-2021年4月1日的42例患者作为观察组,在空肠营养管置入胃内后,不予胃内置入空气。比较2组患者置管时间、置管次数、置管过程中不良反应的发生率及置管成功率。结果对照组置管成功率为84.0%,观察组置管成功率为88.1%,2组患者置管成功率差异无统计学意义(P=0.57);2组患者的置管次数差异无统计学意义(P=0.93);2组患者置管时间比校差异有统计学意义(P=0.04),观察组患者置管时间小于对照组;2组患者置管过程中不良反应的发生率差异有统计学意义(P=0.03),观察组不良反应发生率明显低于对照组。结论对于有创人工气道的危重患者非注气法留置空肠营养管可缩短置管时间,减少呛咳、反流及误吸的发生率,且不影响置管成功率和置管次数。Objective To explore the clinical effect of non-air injection bedside indwelling jejunal nutrition tube in severe patients with invasive artificial airway.Methods A total of 92 patients with invasive artificial airway who needed jejunal nutrition tube were divided into groups from April 2020 to April 2021.Fifty patients who needed jejunal nutrition tube from April 1,2020 to October 1,2020 were taken as the control group,and used the method of conventional jejunal nutrition tube;when the jejunal nutrition tube was placed in the stomach,300-500 mL air was inserted into the stomach to increase the peristaltic capacity of the stomach.Forty-two patients from October 2,2020 to April 1,2021 were taken as the experimental group,and after the jejunal nutrition tube was placed in the stomach,no air was injected into the stomach.The differences in catheterization time,times of catheterization the incidence of adverse reactions,and the success rate of catheterization between the two groups were compared between the two groups.Results The success rate of catheterization was 84.0%in the control group and 88.1%in the experimental group,and there was no significant difference between the two groups(P=0.57).There was no significant difference in the number of catheterization between the two groups(P=0.93),and there was significant difference in catheterization time between the two groups(P=0.04),and the catheterization time in the experimental group was shorter than that in the control group.There was significant difference in the incidence of adverse reactions between the two groups(P=0.03),and the incidence of adverse reactions in the experimental group was significantly lower than that in the control group.Conclusion For critical patients with invasive artificial airway,indwelling jejunal nutrition tube by non-air injection can shorten the catheterization time,reduce the incidence of choking,reflux,and aspiration,and does not affect the success rate and times of catheterization.
