妇科围术期加速康复指南的质量评价

摘　　要：目的对国内外妇科围术期加速康复实践指南进行方法学及报告质量评价,甄别出高质量指南,为临床实践提供参考。方法系统检索Up To Date、BMJ Best Practice、世界卫生组织网站、美国国立指南库、英国国家卫生与临床优化研究所、苏格兰院际指南网、国际指南协作网、加拿大安大略注册护士协会、医脉通指南网,以及PubMed、Embase、CINAHL、Cochrane Library、Web of Science、CNKI、CBM、WanFang、VIP数据库收录的妇科围术期加速康复指南,检索时限为建库至2021年12月20日。采用指南研究与评价工具(AGREEⅡ)和实践指南报告规范(RIGHT)对符合纳入、排除标准的指南分别进行方法学及报告质量评价。结果共纳入妇科围术期加速康复指南6篇,AGREEⅡ评价结果显示:B级推荐5篇,C级推荐1篇,6大领域平均标准化百分比最高为清晰性领域(69.45%),最低为应用性领域(24.30%);RIGHT评价结果显示报告率最高为其他(88.89%)及基本信息领域(86.11%),最低为评审和质量保证领域(16.67%)。结论妇科围术期加速康复指南整体质量一般,国内外指南在制订过程和报告规范方面存在较大差距,条目内容有待细化,质量有待进一步提高。Objective To evaluate the methodology and report quality of domestic and foreign practice guidelines for gynecological perioperative enhanced recovery after surgery,and identify high-quality guidelines,so as to provide reference for clinical practice.Methods Guidelines for gynecological perioperative enhanced recovery after surgery were systematically searched in Up To Date,BMJ Best Practice,WHO,NGC,NICE,SIGN,GIN,RNAO,Medlive,and PubMed,Embase,CINAHL,Cochrane Library,Web of Science,CNKI,CBM,Wanfang,and VIP databases,the retrieval time was from the database establishment to December 20,2021.The AGREEⅡand RIGHT were used to evaluate the methodology and report quality of the guidelines that met the inclusion and exclusion criteria respectively.Results A total of 6 guidelines for gynecological perioperative enhanced recovery after surgery were included.The results of AGREE II evaluation showed that there were 5 pieces of B-level recommendation,and 1 piece of C-level recommendation.The highest average standardized score of the 6 domains was 69.45%for the domain of clarity,and the lowest was 24.30%for the domain of application.The results of RIGHT evaluation showed that the domain with the highest reporting rate was in other(88.89%)and basic information(86.11%),and the lowest in review and quality assurance(16.67%).Conclusion The overall quality of the guidelines for gynecological perioperative enhanced recovery after surgery is general.There is a big gap in the development process and reporting standards of the domestic and foreign guidelines,and the content of the items needs to be refined and the quality needs to be further improved.
