再生障碍性贫血患者血清IL-10、sFas表达水平及临床意义

摘　　要：目的研究再生障碍性贫血患者血清白细胞介素-10(IL-10)、可溶性Fas(sFas)的表达水平及临床意义。方法抽取2017年10月至2021年10月临汾市第二人民医院收治的56例再生障碍性贫血患者为观察组(23例为急性再生障碍性贫血,33例为慢性再生障碍性贫血),另抽取同期于医院行健康体检的50例健康体检者作为对照组。采用酶联免疫吸附试验测定两组血清IL-10、sFas水平。比较两组血清IL-10与sFas水平,比较观察组急性、慢性再生障碍性贫血患者血清IL-10与sFas水平。分析血清IL-10与sFas表达水平的相关性。绘制受试者工作特征曲线,分析血清IL-10、sFas水平对再生障碍性贫血的预测价值。结果观察组血清IL-10水平高于对照组,血清sFas水平低于对照组(P<0.05);观察组急性再生障碍性贫血患者血清IL-10水平高于慢性再生障碍性贫血患者,血清sFas水平低于慢性再生障碍性贫血患者(P<0.05)。Spearman相关性分析结果显示,再生障碍性贫血患者血清IL-10水平与血清sFas水平呈负相关(r=-0.27,P=0.048)。血清IL-10预测再生障碍性贫血的曲线下面积(AUC)、敏感性、特异性分别为0.929、96.40%、64.00%,血清sFas预测再生障碍性贫血的AUC、敏感性、特异性分别为0.910、96.40%、44.00%,血清IL-10与sFas联合预测再生障碍性贫血的AUC、敏感性、特异性分别为0.938、98.20%、64.00%,联合检测的预测值最高。结论再生障碍性贫血患者的血清IL-10呈高表达,血清sFas呈低表达,血清IL-10、sFas水平能对再生障碍性贫血患者的病情严重程度进行客观反映,为临床医师预测再生障碍性贫血发生、病情监测等提供参考。Objective To study the expression and clinical significance of serum interleukin-10(IL-10)and soluble Fas(sFas)in patients with aplastic anemia.Methods Fifty-six patients with aplastic anemia treated in Linfen Second People’s Hospital from October 2017 to October 2021 were selected as the observation group(23 patients with acute aplastic anemia and 33 patients with chronic aplastic anemia),and 50 healthy people who underwent physical examination in the hospital at the same period were selected as the control group.The levels of serum IL-10 and sFas of the two groups were measured by enzyme-linked immunosorbent assay.The levels of serum IL-10 and sFas were compared between the two groups.And the levels of serum IL-10 and sFas were compared between patients with acute and chronic aplastic anemia in the observation group.The correlation between serum IL-10 and sFas expression was analyzed.The receiver operating characteristic curve was drawn to analyze the predictive value of serum IL-10 and sFas levels for aplastic anemia.Results The level of serum IL-10 in the observation group was higher than that in the control group,and the level of serum sFas was lower than that in the control group(P<0.05).The level of serum IL-10 in patients with acute aplastic anemia was higher than that in patients with chronic aplastic anemia,and the level of serum sFas was lower than that in patients with chronic aplastic anemia(P<0.05).Results of Spearman correlation analysis showed that there was a negative correlation between serum IL-10 level and serum sFas level in patients with aplastic anemia(r=-0.266,P=0.048).The area under the curve(AUC),sensitivity and specificity of serum IL-10 in predicting aplastic anemia were 0.929,96.40%and 64.00%,respectively.The AUC,sensitivity and specificity of serum sFas in predicting aplastic anemia were 0.910,96.40%and 44.00%,respectively.The AUC,sensitivity and specificity of serum IL-10 combined with sFas in predicting aplastic anemia were 0.938,98.20%and 64.00%,respectively.The predictive value
