摘  要:目的分析振幅整合脑电图(aEEG)监测对早产儿神经行为发育状况的评估价值。方法抽取郑州市中心医院2019年5月至2020年5月收治的210例早产儿为研究对象,所有早产儿均接受aEEG监测。于纠正胎龄6个月时,记录aEEG评分系统各参数,包括aEEG连续性(Co)、睡眠-觉醒周期(Cy)、下边界值(LB)、带宽(B)、总分(T);另根据新生儿行为神经检查法(NBNA)评估早产儿神经行为发育状况,并将其分为神经行为发育异常组(78例)和神经行为发育正常组(132例)。比较两组早产儿一般资料及纠正胎龄6个月时aEEG参数,分析aEEG监测对早产儿神经行为发育的评估价值。结果神经行为发育异常组Co、Cy、T值均低于神经行为发育正常组,差异有统计学意义(P<0.05);两组其他资料比较差异未见统计学意义(P>0.05)。绘制受试者工作特征曲线图显示,Co、Cy、T值单独及联合评估早产儿神经行为发育异常的曲线下面积分别为0.806、0.791、0.792、0.871,均有一定评估价值。结论aEEG监测对早产儿神经行为发育状况具有一定评估价值。Objective To analyze the value of amplitude integrated electroencephalogram(aEEG)monitoring in the evaluation of neurobehavioral development of preterm infants.Methods A total of 210 premature infants treated in Zhengzhou Central Hospital from May 2019 to May 2020 were selected as research objects,and all premature infants were monitored by aEEG.Every parameter of aEEG scoring system of preterm infants with corrected gestional age of 6 months was recorded,including aEEG continuity(Co),sleep-wake cycle(Cy),lower boundary value(LB),bandwidth(B)and total score(T).And the neurobehavioral development of preterm infant was evaluated according to neonatal behavioral neurological assessment,and they were divided into dysplasia group(78 cases)and well-developed group(132 cases).The general data and aEEG parameters of pretrem infants with corrected gestational age of 6 months were compared between the two groups,the value of aEEG monitoring in the evaluation of neurobehavioral development of preterm infants was analyzed.Results The values of Co,Cy and T in the dysplasia group were lower than those in the well-development group,and the difference was statistically significant(P<0.05);there was no statistical significant difference in comparison of other data between the two groups(P>0.05).The results of receiver operating characteristic curve showed that area under the curve of Co,Cy and T values alone and in combination for evaluating neurobehavioral abnormalities of preterm infants were 0.806,0.791,0.792 and 0.871,respectively,all of which had certain evaluation value.Conclusions aEEG monitoring has certain value in evaluating the neurobehavioral development of preterm infants.
