曲美他嗪联合美托洛尔对老年冠心病伴心力衰竭患者心功能及血清NT-proBNP、hs-CRP水平的影响

摘　　要：目的探讨曲美他嗪联合美托洛尔对老年冠心病伴心力衰竭患者心功能及血清N端脑钠肽前体(NT-proBNP)、超敏-C反应蛋白(hs-CRP)水平的影响。方法抽取2019年5月到2021年5月朔州市人民医院收治的老年冠心病伴心力衰竭患者76例,按随机数字表法分为对照组与观察组,每组38例。对照组行常规治疗,观察组采用曲美他嗪、美托洛尔治疗。比较两组治疗效果、血清NT-proBNP水平、hs-CRP水平及心功能。结果观察组治疗总有效率(94.74%,36/38)高于对照组(76.32%,29/38),P<0.05。治疗后,两组血清NT-proBNP、hs-CRP水平均较治疗前降低,且观察组低于对照组(P<0.05)。治疗后,两组左室射血分数高于治疗前,左室收缩末期内径、左室舒张末期内径低于治疗前,且观察组上述指标优于对照组(P<0.05)。结论老年冠心病伴心力衰竭患者采用曲美他嗪联合美托洛尔治疗效果较好,可有效降低NT-proBNP及hs-CRP水平,改善心功能。Objective To investigate the effects of trimetazidine combined with metoprolol on cardiac function and serum levels of N-terminal pro brain natriuretic peptide(NT-proBNP)and high sensitivity-C-reactive protein(hs-CRP)in elderly patients with coronary heart disease complicated by heart failure.Methods Seventy-six elderly patients with coronary heart disease complicated by heart failure treated in Shuozhou People’s Hospital from May 2019 to May 2021 were divided into control group and observation group by random number table method,with 38 cases in each group.The control group was treated by routine treatment,and the observation group was treated by trimetazidine and metoprolol.The therapeutic effects,serum NT-proBNP level,hs-CRP level and cardiac function of the two groups were compared.Results The total effective rate of the observation group(94.74%,36/38)was higher than that of the control group(76.32%,29/38),P<0.05.After treatment,the levels of NT-proBNP and hs-CRP of the two groups were lower than those before treatment,and the levels of the observation group were lower than those of the control group(P<0.05).After treatment,left ventricular ejection fraction of the two groups were higher than those before treatment,while the left ventricular end systolic diameter and left ventricular end diastolic diameter were lower(P<0.05);and these indexes of the observation group were superior to those of the control group after treatment(P<0.05).Conclusions Trimetazidine combined with metoprolol is effective in the treatment of elderly patients with coronary heart disease complicated by heart failure.It can effectively reduce the levels of NT-proBNP and hs-CRP,and improve cardiac function.
