二甲双胍联合复方醋酸环丙孕酮治疗PCOS疗效及对患者DHEA、AMH、SHBG水平的影响

摘　　要：目的探讨二甲双胍联合复方醋酸环丙孕酮治疗多囊卵巢综合征(PCOS)的疗效及对患者脱氢表雄酮(DHEA)、抗苗勒管激素(AMH)及血清性激素结合球蛋白(SHBG)水平的影响。方法选取100例PCOS患者,根据随机数字表法分为研究组与对照组,每组50例。对照组采用复方醋酸环丙孕酮治疗,研究组在对照组基础上联合二甲双胍治疗。比较两组治疗效果。结果研究组治疗总有效率为94.00%,明显高于对照组的72.00%,差异有统计学意义(P<0.05)。治疗前,两组患者DHEA、AMH及SHBG水平比较,差异无统计学意义(P>0.05);治疗后,两组患者DHEA、AMH水平均降低,SHBG水平升高,且研究组DHEA、AMH水平低于对照组,SHBG水平高于对照组,差异有统计学意义(P<0.05)。研究组患者不良反应发生率明显低于对照组,妊娠率明显高于对照组,差异有统计学意义(P<0.05)。结论二甲双胍联合复方醋酸环丙孕酮治疗PCOS患者疗效显著,可有效改善DHEA、AMH和SHBG水平,降低不良反应发生率,提高患者妊娠率,具有临床应用价值。Objective To investigate the efficacy of metformin combined with compound cyproterone acetate in the treatment of polycystic ovary syndrome(PCOS)and its effect on the levels of dehydroepiandrosterone(DHEA),anti-Mullerian hormone(AMH)and serum sex hormone-binding globulin(SHBG).Methods 100 patients with PCOS were selected and divided into study group and control group according to the random number table method,with 50 cases in each group.The control group was treated with compound cyproterone acetate tablets,and the study group was treated with metformin on the basis of the control group.The treatment effects of the two groups were compared.Results The total effective rate of the study group was 94.00%,which was significantly higher than that of the control group(72.00%),and the diff erence was statistically significant(P<0.05).Before treatment,there was no significant diff erence in the levels of DHEA,AMH and SHBG between the two groups(P>0.05).After treatment,the levels of DHEA and AMH in the two groups were decreased,and the levels of SHBG were increased.The levels of DHEA and AMH in the study group were lower than those in the control group,and the levels of SHBG were higher than those in the control group,and the diff erences were statistically significant(P<0.05).The incidence of adverse reactions in the study group was significantly lower than that in the control group,and the pregnancy rate was significantly higher than that in the control group,and the diff erences were statistically significant(P<0.05).Conclusion Metformin combined with compound cyproterone acetate is effective in the treatment of PCOS patients,which can effectively improve the levels of DHEA,AMH and SHBG,reduce the incidence of adverse reactions,and improve the pregnancy rate of patients,which has clinical application value.
