孕前优生指导配合孕产期规范化干预对高龄孕产妇妊娠结局的影响

摘　　要：目的研究孕前优生指导配合孕产期科学、规范化干预在高龄孕产妇中的应用效果及对妊娠结局的影响。方法选取110例本院建卡的高龄孕产妇,根据孕产期采用的干预方法不同分为对照组与研究组,各55例。研究组采取孕前优生指导配合孕产期规范化干预,对照组采取常规孕期保健。比较两组并发症发生情况、剖宫产、全母乳喂养、产后盆底功能障碍疾病及心理筛查阳性。结果研究组并发症发生率为10.91%,明显低于对照组的34.55%,差异有统计学意义(P<0.05)。研究组剖宫产率、产后盆底功能障碍疾病发生率及心理筛查阳性率均低于对照组,全母乳喂养率明显高于对照组,差异有统计学意义(P<0.05)。结论高龄孕产妇采用孕前优生指导配合系统、全面、个性化的孕期保健,利于并发症预防,改善妊娠结局,降低剖宫产率,提高母乳喂养率,改善负性情绪,促进产后康复,值得大力推广。Objective To study the application effect of pre-pregnancy eugenics guidance combined with scientific and standardized intervention during pregnancy and obstetrics in advanced pregnant women and its impact on pregnancy outcomes.Methods 110 elderly pregnant women were selected and divided into control group and study group according to different intervention methods during pregnancy and perinatal period,55 cases in each group.The study group took prenatal guidance and standardized intervention during pregnancy,while the control group took routine pregnancy health care.The incidence of complications,cesarean section,exclusive breastfeeding,postpartum pelvic floor dysfunction and positive psychological screening were compared.Results The incidence of complications in the study group was 10.91%,which was significantly lower than 34.55%in the control group(P<0.05).The rate of cesarean section,the incidence of postpartum pelvic floor dysfunction and the positive rate of psychological screening in the study group were lower than those in the control group,and the rate of exclusive breastfeeding was significantly higher than that in the control group,with statistical significance(P<0.05).Conclusion Advanced pregnant women adopt pre-pregnancy healthy birth guidance coordination system,comprehensive,personalized pregnancy health care,conducive to the prevention of complications,improve pregnancy outcome,reduce cesarean section rate,improve breastfeeding rate,improve negative emotions,promote postpartum rehabilitation,worthy of promotion.
