不同波长调Q短脉冲激光治疗雀斑样痣的疗效及安全性评估

摘　　要：目的回顾性评估532nm Nd:YAG皮秒激光与Q开关755nm翠绿宝石激光治疗单纯性雀斑样痣的疗效、安全性和影响因素。方法回顾78例采用短脉冲调Q激光治疗的单纯性雀斑样痣患者,根据治疗方案不同分为532nm Nd:YAG皮秒激光治疗组和Q开关755nm翠绿宝石激光治疗组,分析对比两组临床疗效,不良反应以及影响疗效的因素。结果经1~3次治疗,532nm Nd:YAG皮秒激光组与Q开关755nm翠绿宝石激光组总有效率分别为62.5%,52.2%,两者疗效无显著性差异(P>0.05),前者无一例出现不良反应,后者3例(6.5%)出现炎症后色素沉着。疗效与治疗次数成正比,差异有统计学意义(均P<0.05),而性别、首次治疗年龄、发病年龄、发病部位及Fitzpatrick皮肤分型与疗效无相关性。结论532nm Nd:YAG皮秒激光及Q开关755nm翠绿宝石激光治疗单纯性雀斑样痣安全有效,且增加治疗次数可进一步提高患者疗效。Objective The efficacy and safety of 532nm Nd:YAG picosecond laser and Q-switched 755nm alexandrite laser in the treatment of lentigo simplex were evaluated retrospectively.Methods 78 patients with Lentigo Simplex treated with short pulse Q-switched laser were divided into 532nm Nd:YAG picosecond laser treatment group and Q-switched 755nm alexandrite laser treatment group.The clinical efficacy,adverse reactions and influencing factors of the two groups were analyzed and compared.Results After 1 to 3 times of treatment,the total effective rate of 532nm Nd:YAG picosecond laser group and Q-switched 755nm alexandrite laser group was 62.5%and 52.2%respectively,and there was no significant difference between the two groups(P<0.05).There was no adverse reaction in the former and 3 cases(6.5%)in the latter developed post-inflammatory hyperpigmentation.The curative effect was directly proportional to the times of treatment,and the difference was statistically significant(P<0.05),but there was no correlation between sex,age of first treatment,age of onset,location of disease and Fitzpatrick skin classification.Conclusion 532nmNd:YAG picosecond laser and Q-switched 755nm alexandrite laser are safe and effective in the treatment of lentigo simplex,and increasing the frequency of treatment can further improve the efficacy of patients.
