温阳泄浊方对慢性肾衰竭并心衰阳虚水泛患者心功能、肾功能的影响

摘　　要：目的:探讨慢性肾衰竭并心衰患者应用温阳泄浊方的效果。方法:80例慢性肾衰竭心衰阳虚水泛患者按照数字表法随机分为两组,对照组40例,观察组40例,两组患者均给予常规治疗,降压、降脂、利尿、β受体阻滞剂、血管紧张素转化酶抑制剂(ACEI)、醛固酮抑制剂等治疗;观察组在以上治疗的基础上给予温阳泄浊方每日1剂。两组共治疗1月。比较两组心功能、肾功能变化。结果:两组治疗后患者心衰症状均有明显改善(P<0.01,P<0.05),并且观察组与对照组治疗后比较改善更明显(P<0.05)。两组患者治疗后LVEF、LVESd均有明显改善(P<0.01,P<0.05),并且观察组治疗后较对照组治疗后改善更明显(P<0.05)。两组患者治疗后NT-proBNP均有明显降低(P<0.01);并且观察组治疗后较对照组治疗后降低更明显(P<0.05)。观察组的临床治疗总有效率(92.50%)明显高于对照组(75.00%)(χ^(2)=4.02,P<0.05)。两组患者治疗后Scr、BUN均有明显降低(P<0.01,P<0.05),并且观察组治疗后与对照组治疗后比较降低更明显(P<0.05)。结论:慢性肾衰竭并心衰患者服用温阳泄浊方不仅可以改善心功能,而且可以改善肾功能。Objective:To explore the effect of Warming yang and relieving turbidity on patients with chronic renal failure and heart failure. Methods:80 patients with chronic renal failure and heart failure were randomly divided into two groups: control group(40 cases) and observation group(40 cases). The two groups were treated for one month. Cardiac and renal function were were compared between the two groups. Results:After treatment, the symptoms of cardiac function were significantly improved in the two groups(P<0.01, P<0.05), and the improvement was more obvious in the observation group than in the control group(P<0.05). The LVEF and LVESd of the two groups increased significantly after treatment(P<0.01, P<0.05), and the LVEF and LVESd of the observation group improved more significantly after treatment than that of the control group(P<0.05). NT-proBNP of the two groups were significantly reduced after treatment(P<0.01), and the observation group was significantly lower than the control group after treatment(P<0.05). The total effective rate of clinical treatment in the observation group(92.50%) was significantly higher than that in the control group(75.00%)(χ^(2)=4.02, P<0.05). After treatment, SCR and bun of the two groups were significantly reduced(P<0.01, P<0.05), and the observation group was significantly lower than the control group(P<0.05). Conclusion:Warming Yang and relieving turbidity can not only improve heart function, but also improve renal function in patients with chronic renal failure and heart failure.
