摘 要:目的观察不同人乳头瘤病毒(HPV)感染亚型的女性尖锐湿疣患者T淋巴细胞亚群表达,并分析T淋巴细胞亚群与疾病复发的关系。方法选择162例女性尖锐湿疣患者进行回顾性病例对照研究,将其中81例治疗后复发的患者纳入复发组,81例治疗后未复发患者纳入未复发组。采用独立样本t检验比较不同HPV感染亚型患者T淋巴细胞亚群表达;采用Kendall’s tau-b相关性检验分析HPV感染亚型与T淋巴细胞亚群的相关性。采用χ^(2)检验、t检验比较复发与未复发组患者基线资料、HPV感染亚型及T淋巴细胞表达;采用Logistics回归分析T淋巴细胞亚群表达对女性尖锐湿疣患者复发的影响。结果HPV感染高危型患者CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)水平低于低危型(P<0.05);不同HPV感染亚型CD8^(+)比较,差异无统计学意义(P>0.05)。经Kendall’s tau-b相关性分析检验,结果显示HPV感染亚型与CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)水平呈负相关(r<0,P<0.05)。复发组大量饮酒史、合并泌尿系统感染占比高于未复发组,使用安全套占比低于未复发组(P<0.05);复发组CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)低于未复发组(P<0.05)。经Logistics回归分析,结果显示HPV感染高危型是尖锐湿疣复发的危险因素(OR>1,P<0.05),CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)高是尖锐湿疣复发的保护因素(OR<1,P<0.05)。结论不同HPV感染亚型女性尖锐湿疣患者T淋巴细胞表达差异显著,且T淋巴细胞亚群表达与疾病复发密切相关。Objective To observe the expression of T lymphocyte subsets in female condyloma acuminatum patients with different subtypes of human papillomavirus(HPV)infection and analyze the relationship between T lymphocyte subsets and the disease recurrence.Methods A retrospective case-control study was conducted on the 162 female condyloma acuminatum patients admitted to the Tongji Hospital Affiliated to the Tongji Medical College of the Huazhong University of Science and Technology.Among them,81 patients with relapse after treatment were included in the relapse group,while 81 patients without relapse after treatment were included in the non-relapse group.The expression of T lymphocyte subsets in patients with different HPV infection subtypes was compared by the independent sample t test.The Kendall’s tau-b correlation test was used to analyze the correlation between HPV infection subtypes and T lymphocyte subsets.Theχ^(2) test and t test were used to compare the baseline data,HPV infection subtype and T lymphocyte expression of patients in the relapse and non-relapse groups.Logistic regression was used to analyze the effect of T lymphocyte subsets expression on the recurrence of female condyloma acuminatum patients.Results The levels of CD3^(+),CD4^(+),CD4^(+)/CD8^(+)in the high-risk HPV patients were lower than those in the low-risk HPV patients(P<0.05).There was no significant difference in CD8^(+)among different HPV infection subtypes(P>0.05).The Kendall’s tau-b correlation analysis showed that HPV infection subtypes were negatively correlated with the levels of CD3^(+),CD4^(+),CD4^(+)/CD8^(+)(r<0,P<0.05).The proportion of heavy drinking history and urinary system infection in the relapse group was higher than that in the non-relapse group,and the proportion of condom use was lower than that in the non-relapse group(P<0.05).The levels of CD3^(+),CD4^(+),CD4^(+)/CD8^(+)in the relapse group were lower than those in the non-relapse group(P<0.05).The results of Logistic regression analysis showed that the high-risk ty
