体外冲击波经络腧穴法联合消瘀散治疗老年胸腰椎骨折PKP术后腰痛的效果观察

摘　　要：目的观察体外冲击波经络腧穴法联合消瘀散外敷治疗老年胸腰椎骨折经皮椎体后凸成型术(PKP)后残留腰痛的疗效。方法58例老年胸腰椎骨折PKP术后残留腰痛患者,随机分为治疗组与对照组,各29例。对照组采用体外冲击波经络腧穴法治疗,治疗组在对照组的基础上辅以中药消瘀散外敷。比较两组治疗后的视觉模拟评分法(VAS)评分及疗效。结果治疗后1、2、4周,治疗组VAS评分分别为(2.69±0.81)、(1.34±0.61)、(1.10±0.49)分,均低于对照组的(3.78±0.80)、(2.96±1.09)、(2.41±0.64)分,差异有统计学意义(P<0.05);治疗后8周,两组VAS评分比较,差异无统计学意义(P>0.05)。治疗组治疗优良率为100.00%,高于对照组的65.52%,差异有统计学意义(P<0.05)。结论体外冲击波经络腧穴法联合中药消瘀散外敷治疗老年胸腰椎骨折PKP术后残留腰痛安全有效,能快速缓解疼痛,促进腰部功能恢复。Objective To observe the efficacy of extracorporeal shock wave meridian acupoints combined with Xiaoyu powder in the treatment of low back pain after percutaneous kyphoplasty(PKP)for thoracolumbar fractures in the elderly.Methods A total of 58 elderly patients with residual low back pain after PKP for thoracolumbar fractures were randomly divided into treatment group and control group,with 29 cases in each group.The control group was treated with extracorporeal shock wave meridian and acupoint,and the treatment group was supplemented with external application of traditional Chinese medicine Xiaoyu powder on the basis of the control group.The visual analogue scale(VAS)score after treatment and clinical efficacy were compared between the two groups.Results At 1,2,and 4 weeks after treatment,the VAS scores in the treatment group were(2.69±0.81),(1.34±0.61),and(1.10±0.49)points,which were lower than(3.78±0.80),(2.96±1.09),and(2.41±0.64)points in the control group,and the differences were statistically significant(P<0.05).At 8 weeks after treatment,there was no statistically significant difference in VAS score between the two groups(P>0.05).The excellent and good rate in the treatment group was 100.00%,which was higher than 65.52%in the control group,and the difference was statistically significant(P<0.05).Conclusion Extracorporeal shock wave meridian acupoints combined with Xiaoyu powder is safe and effective in the treatment of low back pain after PKP for thoracolumbar fractures in the elderly,and can quickly relieve pain,promote the recovery of low back function.
