奥布卡因表面麻醉联合利多卡因前列腺尖部神经阻滞麻醉对经直肠前列腺穿刺活检的临床研究

摘　　要：目的对比分析奥布卡因表面麻醉联合利多卡因前列腺尖部神经阻滞麻醉与单纯奥布卡因表面麻醉对于经直肠前列腺穿刺术中疼痛的控制效果和穿刺后并发症发生情况,为两种麻醉方法在经直肠前列腺穿刺术合理应用提供技术支持。方法回顾性分析我院2018年1月至2020年12月行10~16针前列腺穿刺活检患者共200例,即单纯奥布卡因表面麻醉组(对照组)和奥布卡因表面麻醉联合利多卡因前列腺尖部神经阻滞麻醉组(试验组),收集两组患者的一般人口学信息、穿刺时间、长海痛尺法疼痛评分、穿刺后并发症发生情况,采用χ^(2)检验、t检验等方法进行统计分析。结果试验组的疼痛评分低于对照组,差异有统计学意义(t=3.193,P=0.002),且他们之间的疼痛程度分布不同,即试验组的轻度疼痛比例高于对照组,差异有统计学意义(Z=7.824,P<0.001);试验组穿刺时间稍长于对照组(t=20.093,P<0.001);但两组穿刺术后肉眼血尿、血便、发热、尿潴留和血精并发症的发生率差异无统计学意义(χ^(2)=0.125,P=0.86)。结论奥布卡因表面麻醉联合利多卡因前列腺尖部神经阻滞麻醉的安全有效、镇痛效果确切,不会增加麻醉并发症,适用于经直肠超声引导下前列腺系统+融合靶向穿刺活检常规应用。Objective To compare and analyses the control effect of topical anesthesia combined with lidocaine for apical prostatic nerve block and topical anesthesia only with ocbuvacaine for pain control and complications after puncture,so as to provide technical support for rational application of the two anesthesia methods in transrectal prostate puncture.Methods Retrospective analysis of our hospital from January 2018 to December 2020,line 10~16,a total of 200 patients with needle biopsy of the prostate,namely pure oxybuprocaine surface anesthesia group(control group)and oxybuprocaine surface anesthesia combined lidocaine prostatic apex of nerve block anesthesia group(experimental group),two groups of patients with general demographic information collection,puncture time,changhai pain feet pain score,puncture complications occurred after,χ^(2) test and t test methods for statistical analysis.Results The pain score of the experimental group was lower than that of the control group(t=3.193,P=0.002),and the distribution of pain degree was different between them,that is,the proportion of mild pain in the experimental group was higher than that in the control group(Z=7.824,P<0.001).The puncture time of experimental group was slightly longer than that of control group(t=20.093,P<0.001).However,there were no statistical significantly differences in the incidence of gross hematuria,hemopoecia,fever,urinary retention and hemospermia complications between the two groups(χ^(2)=0.125,P=0.86).Conclusion Obuvacaine surface anesthesia combined with lidocaine apical prostatic nerve block anesthesia is safe,effective,accurate analgesic effect,will not increase anesthesia complications,suitable for transrectal ultrasonic-guided prostate system add fusion targeted puncture biopsy routine application.
