摘 要:目的:探讨双低技术基于多模型迭代重建算法在改善肝脏CT增强检查中图像质量及辐射剂量的价值。方法:回顾性收集60例肝脏增强患者作为研究对象,根据随机数字表法分为A组和B组,每组30例。A组管电压80kVp,碘浓度为300mgI/mL碘海醇对比剂,采用多模型迭代重建算法(ASiR-V)重建;B组管电压120kVp,碘浓度350mgI/mL碘佛醇对比剂,采用滤波反投影算法(FBP)重建。比较两组图像质量客观评价指标[肝脏CT值、噪声(SD)、信噪比(SNR)及对比噪声比(CNR)]和主观评价指标(图像噪声、图像失真度及图像诊断评分),并计算两组患者有效辐射剂量及碘摄入量。结果:与B组比较,A组患者有效辐射剂量降低了39.93%,碘摄入量减少了44.90%,SD降低了42.93%,SNR及CNR分别提高了40.70%、49.42%,差异均有统计学意义(P<0.05)。两组图像客观评价指标噪声评分、失真度评分及诊断评分比较差异均无统计学意义(P>0.05),且2名观察者之间评价一致性较好(Kappa=0.843)。结论:在肝脏CT增强检查中,与常规管电压FBP算法相比,低管电压低浓度对比剂结合ASiR-V算法可显著降低患者有效辐射剂量及碘摄入量,且能够得到满意的图像质量,临床价值较高,值得推广应用。Objective:Discuss the value of double low technique based iterative reconstruction algorithm in improving image quality and radiation dose in liver CT enhancement examination.Methods:Retrospective collection of 60 patients with liver enhancement, they were divided into A group and B group according to the random number table method, each group was 30 cases.A group of tube voltage 80 kVp, iodine concentration is 300 mgI/mL iodohydanol contrast agent, Multi-model iterative reconstruction algorithm(ASiR-V) reconstruction, B group of tube voltage 120, iodine concentration 350 mgI/mL iodophorol contrast agent, filter backprojection algorithm(FBP) reconstruction.The objective evaluation indexes of image quality [liver CT value, noise(SD), signal-to-noise ratio(SNR) and contrast noise ratio(CNR)] and subjective evaluation indexes(image noise, image distortion and image diagnosis score) were compared between the two groups, and the effective radiation dose and iodine intake were calculated.Results:The effective radiation dose decreased by 39.93%, iodine intake decreased by 44.90%, SD decreased by 42.93%, and SNR and CNR increased by 40.70% and 49.42%, respectively, in A group(P<0.05).There was no significant difference in noise score, distortion score and diagnostic score between the two groups(P>0.05), and the evaluation consistency between the two observers was good(Kappa=0.843).Conclusion:The combination of low tube voltage and low concentration contrast agent and ASiR-V algorithm can significantly reduce the effective radiation dose and iodine intake of patients and can obtain satisfactory image quality compared with the conventional tube voltage FBP algorithm. The clinical value is high is worth popularizing and applying.
