摘 要:目的研究外周血可溶性髓样细胞触发性受体-1(sTREM-1)、可溶性白细胞介素-2受体(sIL-2R)及瘦素(leptin)水平与新生儿肺炎(NP)病情程度及预后的相关性。方法选择本院2017年4月—2020年6月诊治的109例NP,均检测外周血sTREM-1、sIL-2R及leptin水平。根据患儿病情程度分为轻症组和重症组,分析不同病情患儿sTREM-1、sIL-2R、leptin水平及与病情程度的关系;根据患儿预后情况分为预后不良组和预后良好组,分析不同预后患儿sTREM-1、sIL-2R、leptin水平及对预后的预测效能,并分析上述指标水平与预后的关系。结果重症组患儿sTREM-1、sIL-2R水平显著高于轻症组,leptin水平显著低于轻症组(P<0.01)。预后良好组患儿sTREM-1、sIL-2R水平显著低于预后不良组,leptin水平显著高于预后不良组(P<0.01)。sTREM-1、sIL-2R联合leptin预测NP患儿预后的受试者工作特征曲线下面积及特异度均高于单一指标预测。sTREM-1、sIL-2R及leptin水平均是NP患儿预后不良的独立危险因素(P<0.05,P<0.01)。结论外周血sTREM-1、sIL-2R及leptin水平与NP患儿病情严重程度相关,是影响患儿预后的重要因素,其水平高低可作为评估患儿病情、预测预后的重要指标,且三者联合检测可提高对预后的预测效能。Objective To study the correlation between the levels of soluble myeloid cell trigger receptor-1(sTREM-1),soluble interleukin-2 receptor(sIL-2R)and leptin in peripheral blood and the severity and prognosis of neonatal pneumonia(NP).Methods A total of 109 cases of NP diagnosed and treated in our hospital from April 2017 to June 2020 were selected.The levels of sTREM-1,sIL-2R and leptin in peripheral blood were detected.According to the severity of illness,the children were divided into mild illness group and severe illness group.The levels of sTREM-1,sIL-2R,leptin and their relationship with the severity of illness were analyzed.According to the prognosis of children,they were divided into poor prognosis group and good prognosis group.The levels of sTREM-1,sIL-2R,leptin in children with different prognosis and their predictive efficacy for prognosis were analyzed,and the relationship between the levels of the above indicators and prognosis was analyzed.Results The levels of sTREM-1 and sIL-2R in the severe group were significantly higher than those in the mild group,and the levels of leptin were significantly lower than those in the mild group(P<0.01).The levels of sTREM-1 and sIL-2R in the good prognosis group were significantly lower than those in the poor prognosis group,and the levels of leptin were significantly higher than those in the poor prognosis group(P<0.01).The area under the ROC curve and specificity of sTREM-1,sIL-2R combined with leptin in predicting the prognosis of children with NP were higher than those predicted by a single index.The levels of sTREM-1,sIL-2R and leptin were independent risk factors for poor prognosis in children with NP(P<0.05,P<0.01).Conclusion The levels of sTREM-1,sIL-2R and leptin in peripheral blood are related to the severity of NP children,and are important factors affecting the prognosis of NP children.Their levels can be used as important indicators to evaluate the condition and predict the prognosis of NP children,and their combined detection can improve the predictiv
