摘 要:目的:探讨等速肌力视觉反馈训练对半月板损伤患者关节镜术后膝关节肌力、本体感觉和关节功能康复的影响。方法:选取2019年7月至2021年12月首次关节镜手术在海军青岛特勤疗养中心进行康复治疗的患者150例,随机分组为对照组(n=75)与观察组(n=75)。对照组术后予以常规康复训练,观察组在常规康复基础上实施等速肌力视觉反馈训练,两组康复治疗均持续3个月。于康复治疗前和治疗后,采用膝关节国际通用评分(International Knee Documentation Committee,IKDC)、Lysholm评分对患者膝关节功能进行评价,并对比两组膝关节肌力和本体感觉情况(平均轨迹误差、平均负重力量差)。结果:观察组优良率为78.67%,相比对照组的61.33%提高,差异有统计学意义(P<0.05)。治疗后,观察组IKDC、Lysholm评分均高于对照组,差异有统计学意义(均P<0.05)。治疗后,观察组膝关节屈、伸肌峰力距均明显大于对照组(均P<0.05),平均轨迹误差、平均负重力量差均明显小于对照组(均P<0.05)。结论:等速肌力视觉反馈训练能够改善关节镜术后半月板损伤患者膝关节肌力和本体感觉,有助于促进膝关节功能恢复,是有效的术后康复训练方法。Objective: To investigate the effect of muscle isokinetic training with visual biofeedback on knee joint muscle strength, proprioception and joint function in patients with meniscus injury after arthroscopic surgery. Methods: A total of 150 patients who underwent the first arthroscopic surgery in Qingdao Special Service Rehabilitation Center, PLA Navy from July 2019 to December 2021 were randomly divided into control group(n=75) and observation group(n=75). The control group received routine rehabilitation training a?ter operation, and the observation group received muscle isokinetic training with visual biofeedback on the basis of routine rehabilitation, rehabilitation treatment lasted for 3 months in both groups. Before and a?ter rehabilitation treatment, knee function was evaluated by The Pedi International Knee Documentation Committee(IKDC) and Lysholm score, and knee muscle strength and proprioception(mean trajectory error and mean weight-bearing strength difference) were compared between the 2 groups. Results: The excellent and good rate of the observation group was 78.67%, which was higher than 61.33 % of the control group, and the difference was statistically significant(P<0.05). A?ter treatment, the IKDC and Lysholm scores in the observation group were higher than those in the control group, and the difference was statistically significant(both P<0.05). A?ter treatment, the peak force distance of knee flexion and extension in the observation group was significantly greater than that in the control group(both P<0.05), and the average trajectory error and average weight bearing force difference were significantly less than those in the control group(both P<0.05). Conclusion: Muscle isokinetic training with visual biofeedback can improve the muscle strength and proprioception of knee joint in patients with meniscus injury a?ter arthroscopic surgery and promote the recovery of knee joint function, which is an effective postoperative rehabilitation training method.
