摘 要:目的评估容积再现(VR)成像在鼻区线性骨折中的辅助价值.方法纳入100例鼻区线性骨折和70例无骨折的鼻骨CT图像,包括原始图像、多平面重建(MPR)图像(横断面和冠状面)和VR图像.3名不同级别医师以盲法分别采用MPR图像、MPR联合VR图像、MPR联合原始图像对鼻区有无骨折进行评估.结果医师A对鼻骨和上颌骨额突骨折的诊断效能在MPR图像、MPR联合VR图像和MPR联合原始图像三者总体比较均有统计学差异(P<0.001),MPR联合VR图像对鼻骨和上颌骨额突骨折的诊断效能比单独MPR图像的诊断效能均有明显提高(P<0.05);医师B对鼻骨骨折的诊断效能在三者总体比较有统计学差异(P<0.05),对上颌骨额突骨折的诊断效能在三者总体比较无统计学差异(P>0.05);医师C对鼻骨和上颌骨额突骨折的诊断效能在三者总体比较均无统计学差异(P>0.05).3名医师在MPR联合VR图像阅片用时比MPR联合原始图像阅片用时显著缩短(P<0.001).结论MPR联合VR图像可提高低级别医师在鼻区线性骨折的诊断效能,同时缩短阅片时间.ObjectiveTo evaluate the assistance effectiveness of volume rendering(VR)imaging in the clinical diagnoses of linear fracture of nasal bone area.Methods The CT images of nasal bone of 100 patients with linear nasal fractures and 70 patients without fractures were collected,including original images,multiplanar reconstruction(MPR)(axial and coronal images),and VR images.The MPR images,MPR combined with VR images,MPR combined with original images of nasal bone fractures were then evaluated individually by three radiologists with different seniority with blinding.Results There were significant differences in the detections of nasal bone fracture and the frontal process of the maxila fracture among radiologist A with MPR,radiologist A with MPR and VR,and radiologist A with MPR and original images(P<0.001).The detection performances of nasal bone fracture and the frontal process of the maxilla fracture of radiologist A with MPR and VR were significantly better than that of radiologist A with MPR(P<0.05).The diagnostic performances of radiologist B in nasal bone fracture were statistically different among the three detection methods(P<0.05).The diagnostic efficacy of radiologist B in the frontal process of the maxilla fracture was not statistically different among the three detections(P>0.05).No significant difference in the detections of nasal bone fracture and the frontal process of the maxilla fracture among radiologist C using three detection methods was found(P>0.05).With the assistance of MPR and VR,the diagnostic times of three radiologists were much shorter than which with assistance of MPR and original image(P<0.o01).Conclusion MPR combined with VR image can improve the linear fracture of nasal bone area diagnostic efectiveness of junior radiologists and shorten the diagnostic time.
