摘  要:目的:分析常规2D-PDWI(proton density weighted imaging)和高分辨PDWI对踝关节外侧韧带的显示效果,为临床上踝关节外侧韧带损伤的诊断提供更准确的依据。材料与方法:应用3.0T磁共振对21名正常志愿者踝关节分别行常规2D-PDWI-TSE、高分辨PDW-TSE扫描后多平面重建,由2名资深MR医生对距腓前韧带、距腓后韧带、跟腓韧带的图像质量(模糊、假象、容积效应、均匀性)以及扫描时间两个方面用5分利克特量表(5-point Likert scale)进行评估。结果:3.0T高分辨2D-PDWI-TSE扫描多平面重建图像对距腓前韧带、跟腓韧带显示效果优于常规2D-PDWI-TSE图像,两者差别具有统计学意义(P<0.01),但两种序列扫描在距腓后韧带的显示上无统计学意义(P>0.05)。高分辨PDW-TSE扫描时间比常规2D-PDWI时间少52s。结论:3.0T高分辨PDWI-TSE多平面重建图像对踝关节外侧副韧带的显示明显优于常规2D-PDWI-TSE图像,且扫描速度更快,能更好地应用于临床评估踝关节外侧副韧带损伤。Objective:To analyze the display effect of conventional 2 D-PDWI(proton density weighted imaging)and high resolution PDWI on lateral ankle ligament,and to provide more accurate basis for clinical diagnosis of lateral ankle ligament injury.Materials and methods:Conventional 2 D-PDWI-TSE and high-resolution PDW-TSE scanning after multi-planar reconstruction examination were performed on the ankle joints of 21 normal volunteers using 3.0 T magnetic resonance imaging.Two senior MR surgeons evaluated the image quality(blur,artifact,volume effect,uniformity)and scan time of the anterior talofibular ligament,posterior tibiofibular ligament,and patellofemoral ligament by using a 5-point Likert scale.Results:3.0 T high-resolution 2 D-PDWI-TSE multi-planar reconstruction images showed anterior talofibular ligament and calcaneofibular ligament better than conventional 2 D-PDWI-TSE images,the difference was statistically significant(P<0.01),but there was no statistical significance in the display of posterior talofibular ligament between the two sequential scans(P>0.05).The scanning time of high resolution PDW-TSE was 52 s less than that of conventional 2 D-PDWI.Conclusion:The 3.0 T high resolution PDWI-TSE multi-plane reconstruction image is superior to the conventional 2 D-PDWI-TSE image in displaying the lateral collateral ligament of the ankle,and the scanning speed is faster,which can be better applied to the clinical evaluation of the lateral collateral ligaments of the ankle joint.
