某院住院患者硫酸庆大霉素注射剂超说明书用药情况调查分析

摘　　要：目的 调查某院住院患者硫酸庆大霉素注射剂超说明书用药情况,为临床合理用药提供参考。方法 采用等距抽样法抽取医院2017年5月至2020年1月使用硫酸庆大霉素注射剂的住院患者910例,最终纳入853例,涉及医嘱921条。根据药品说明书判断医嘱是否为超说明书用药,并查阅国内外说明书、指南、文献等对超说明书用药进行分析。结果 921条医嘱中,硫酸庆大霉素注射剂超给药途径用药919条(99.78%),其中超适应证用药576条(62.54%),涉及7种超给药途径及多种适应证,且相应的用法用量较为混乱。在超给药途径用药分布中,雾化给药和手术冲洗分别占64.31%、27.42%。超给药途径用药排名前3的科室为耳鼻喉科、眼科、普通外科,共计占总数的62.46%。结论 该院硫酸庆大霉素注射剂超说明书用药现象普遍;与国内外报道数据相比,该院的硫酸庆大霉素注射剂超说明书用药质量不高,医院应规范该药的超说明书用药,保证患者安全用药。Objective To investigate the off-label use of gentamicin sulfate injection in the inpatients of a hospital,so as to provide reference for clinical rational drug use.Methods A total of 910 inpatients using gentamicin sulfate injection from May 2017 to January 2020 were selected by isometric sampling,and 853 inpatients were finally included,involving921 medical records.Whether the medical record was off-label use was judged according to the drug instructions,and the off-label uses were analyzed by reviewing domestic and foreign drug instructions,guidelines and literatures.Results Of the921 medical records involving gentamicin sulfate injection,919(99.78%) were off-label route of administrations,576(62.54%) of them were off-label indications,involving 7 routes of administrations and multiple indications,and the corresponding use was confusing.In the distribution of off-label route of administration,atomization administration and surgical irrigation accounted for 64.31% and 27.42% respectively.The top 3 departments in terms of off-label route of administration were otolaryngology,ophthalmology and general surgery departments,accounting for 62.46% of the total.Conclusion Off-label use of gentamicin sulfate injection is common in this hospital;compared with the data reported at home and abroad,the quality of the off-label use of gentamicin sulfate injection in this hospital is not high,and the hospital should standardize the off-label use of this drug to ensure the safe use of patients.
