摘　　要：目的 探讨关节镜下肩袖修复术治疗肩袖损伤的效果及对肩关节疼痛程度、活动情况的影响。方法 选取我院收治的86例肩袖损伤患者作为研究对象,根据手术方案差异将其分为常规组(43例,小切口肩袖修复术)和关节镜组(43例,关节镜下肩袖修复术)。比较两组的治疗效果。结果 术后4周,关节镜组的美国加州大学肩关节评分(UCLA)、美国肩肘外科协会(ASES)评分显著高于常规组,视觉模拟评分法(VAS)评分显著低于常规组(P<0.05)。术后4周,关节镜组的患肩关节外展90°外旋、外展90°内旋、体侧外旋、外展及前屈的活动度均显著大于常规组(P<0.05)。术后3 d,关节镜组的P物质(SP)、β-内咖肽(β-EP)、神经生长因(NGF)、肿瘤坏死因子-α(TNF-α)、超敏C反应蛋白(hs-CRP)、白细胞介素6(IL-6)水平均显著低于常规组(P<0.05)。结论 关节镜下肩袖修复术治疗肩袖损伤的效果显著,可减轻术后疼痛程度、应激刺激及炎症反应,促进肩关节功能恢复。Objective To investigate the effect of arthroscopic rotator cuff repair on rotator cuff injury and its influence on pain degree and movement of shoulder joint.Methods A total of 86 patients with rotator cuff injury admitted in our hospital were selected as the research objects and divided into conventional group(43 cases,small incision rotator cuff repair) and arthroscopic group(43 cases,arthroscopic rotator cuff repair) according to the difference of surgical schemes.The therapeutic effects of the two groups were compared.Results Four weeks after operation,the University of California at Los Angeles Shoulder Rating Scale(UCLA) and the American Shoulder and Elbow Surgeons(ASES) scores in the arthroscopic group were significantly higher than those in the conventional group,and the Visual Analogue Scale(VAS)score was significantly lower than that in the conventional group(P<0.05).Four weeks after operation,the range of motion of shoulder joint abduction 90° external rotation,abduction 90° internal rotation,lateral external rotation,abduction and forward flexion in the arthroscopic group were significantly greater than those in the conventional group(P<0.05).Three days after operation,the levels of substance P(SP),β-endorphin(β-EP),nerve growth factor(NGF),tumor necrosis factor-α(TNF-α),high-sensitivity C reactive protein(hs-CRP) and interleukin 6(IL-6) were significantly lower than those in the conventional group(P <0.05).Conclusion Arthroscopic rotator cuff repair in the treatment of rotator cuff injury has a significant effect,which can reduce the postoperative pain degree,stress stimulus and inflammatory reaction,and promote the recovery of shoulder joint function.
