土黄连外洗液对乳腺癌急性放射性皮炎的疗效观察

摘　　要：目的观察土黄连外洗液湿敷对乳腺癌急性放射性皮炎的临床疗效。方法采用随机对照研究方法,对符合纳入标准的70例乳腺癌术后拟放疗患者进行随机分组,其中试验组36例、对照组34例,对照组患者予常规护理和健康宣教,试验组则在对照组基础上予土黄连外洗液湿敷放射区域皮肤。比较两组患者放疗3周内的RTOG分级、急性放射性皮炎发生率、皮肤疼痛视觉模拟评分(Visual Analogue Score,VAS)、皮肤瘙痒视觉模拟评分(VAS)、红斑程度及不良反应。结果本研究试验组纳入患者36例,对照组纳入31例。放疗第14、21天,在RTOG分级方面,试验组的RTOG分级均低于对照组,差异均有统计学意义(P<0.05);试验组的急性放射性皮炎发生率均低于对照组,差异均有统计学意义(P<0.05)。试验组患者放射区域皮肤的红斑程度、皮肤疼痛视觉模拟评分、皮肤瘙痒视觉模拟评分均低于对照组,差异有统计学意义(P<0.05)。两组患者试验前后均未出现相关不良反应。结论乳腺癌患者放疗期间使用土黄连外洗液可降低急性放射性皮炎的发生率及其严重程度,有效延缓红斑、疼痛及瘙痒的产生及进展,且均未出现不良反应。Objective To observe the clinical effects of Tuhuanglian lotion wet compress on acute radiation dermatitis of breast cancer patients.Methods By using the method of clinical randomized controlled study,70 cases of breast cancer patients meeting the inclusion criteria and planning to perform postoperative radiotherapy were randomly divided into two groups.The trial group was assigned 36 cases and the control group was assigned 34 cases.The control group was given routine nursing and health training.On the basis of the treatment for the control group,the trial group was given wet compress of Tuhuanglian lotion on the skin of the radiation area.The RTOG grade,incidence of acute radiation dermatitis,visual analog scale(VAS)scores of skin pain and itching,erythema degree,and the incidence of adverse reactions were compared between the control group and the trial group within 3 weeks after the initiation of RT.Results Eventually,a total of 36 cases in the trial group and 31 cases in the control group completed the trial.On the 14th and 21st day of radiotherapy,the RTOG grade of the trial group was lower than that of the control group,and the difference was statistically significant(P<0.05).The incidence of radiation dermatitis in the trial group was lower than that of the control group,and the difference was statistically significant(P<0.05).At each time points of observation,the degree of erythema in the skin of the radiation area,VAS scores of skin pain and itching of the trial group was lower than that of the control group,and the difference was statistically significant(P<0.05).There were no related adverse reactions occurring in both groups.Conclusion The application of Tuhuanglian lotion during radiotherapy for breast cancer patients can reduce the incidence and the severity of acute radiation dermatitis,and relieve erythema,pain and pruritus effectively.No adverse reactions occur during the treatment.
