摘  要:目的探讨改良食物交换份法联合家庭主要成员烹饪实践教育对肾病腹膜透析患者营养状况的影响。方法2021年1—9月,选取河南省某三级甲等医院肾内科病区收治的腹膜透析患者154例,按照组间基本特征具有可比性的原则分为A组(51例)、B组(50例)和C组(53例)。A组采用常规饮食指导,B组采用改良肾病食物交换份法饮食指导,C组采用改良肾病食物交换份法联合家庭主要成员烹饪实践教育饮食指导。比较干预前和干预3个月后3组间3 d饮食日记计算结果、营养相关实验室指标和营养状况。结果干预3个月后,3 d饮食日记计算结果显示B组较A组蛋白质摄入量增加,C组较A组和B组蛋白质摄入量增加,钠、钾、磷摄入量减少,差异具有统计学意义(P<0.05)。干预后,营养相关实验室指标结果显示B组较A组血清白蛋白增高;C组较A组和B组血清白蛋白增高,血磷降低,差异具有统计学意义(P<0.05)。营养状况结果显示C组较A组和B组营养正常比例增高,差异有统计学意义(P<0.05)。结论改良肾病食物交换份法联合家庭主要成员烹饪实践教育应用到腹膜透析患者的饮食指导中可更好地促使患者形成良好的饮食行为,改善患者的营养状况。Objective To explore the ef fects of improved food exchange method combined with cooking practice education of main family members on nutritional status of peritoneal dialysis patients with renal disease.Methods To select 154peritoneal dialysis patients admitted to the nephrology ward of a tertiary A-level hospital in Henan Province from January to September 2021 as the study objects,and they were divided into three groups,such as group A(51 cases),group B(50 cases)and C group(53 cases)according to the principle of comparability of basic characteristics between groups.Group A was given routine dietary guidance,group B was given dietary guidance by the modifi ed kidney disease food exchange portion method,and group C was given the improved kidney disease food exchange portion method combined with the main family members’cooking practice education dietary guidance.The 3-day dietary diary calculation results,nutrition-related laboratory indicators and nutritional status were compared between the three groups before the intervention and after 3 months of the intervention.Results After 3 months of intervention,the 3-day dietary diary calculation results showed that the protein intake of group B increased compared with group A,the protein intake of group C increased compared with groups A and B,and the intake of sodium,potassium and phosphorus decreased,and the dif ferences were statistically signifi cant(P<0.05).After intervention,the results of nutrition-related laboratory indicators showed that the serum albumin in group B was higher than that in group A,and the serum albumin in group C was higher than that in groups A and B,and serum phosphorus was lower,and the differences were statistically significant(P<0.05).The results of nutritional status showed that the proportion of normal nutrition in group C was higher than that in groups A and B,and the dif ference was statistically signifi cant(P<0.05).Conclusion The application of the improved nephropathy food exchange method combined with the cooking practice educa
