摘 要:目的:分析RevolutionCT低keV(40-70 keV)单能量在应用全新迭代算法(ASiR-V)基础上对腹部血管图像质量的影响,探讨低keV改善腹部动静脉血管图像的临床价值。方法:搜集行腹部检查的患者48例,应用CT机常规使用的40%迭代算法进行能谱增强扫描,重建获得40-140 keV(间隔10 keV)11组单能量图像及常规120 kVp-like图像,分析比较各低keV图像及常规120 kVp-like图像门静脉(PV)、肠系膜上静脉(SMV)、肠系膜上动脉(SMA)、腹主动脉(AA)的CT值、SD值和对比信噪比(CNR),并进行主观评分,采用ANOVA单因素方差分析和Wilcoxon秩和检验对数据进行统计学分析。结果:60 keV单能量(4.64±0.68)对于腹部血管显示诊断效能高于常规120 kVp-like图像(3.93±0.60)(P<0.05),40 keV图像血管-肝实质对比度增加,SD值明显增大,整体图像粗糙,观察明显受限;大于60 keV单能量,增强效果逐渐变差;各血管在40-70 keV单能量图像的CT值、CNR和SD值随着单能量的递增而减小,40-70 keV单能量的CT值高于常规120 kVp-like图像(P<0.05),增强效果优于常规120 kVp-like;40-70 keV单能量的CNR值均高于常规120 kVp-like图像(P<0.05)。系统默认最佳单能量40 keV的CNR与60 keV明显差异(P<0.05);能谱低keV单能量的SD值均高于常规120 keV-like,但50-70 keV单能量图像可达到影像诊断要求。结论:RevolutionCT能在应用ASiR-V的条件下对腹部血管不同低keV单能量进行评价,并且单能量对腹部血管显示明显优于常规120 kVp-like图像,且最佳单能量为60 keV。Objective:To explore improving the image quality of abdominal artery and vein clinical value of Revolution CT low keV(40-70keV)single energy conbined with the adaptive statistical iterative reconstruction-V algorithm(ASiR-V).Methods:48 patients underwent abdominal energy spectrum CT enhancement based on 40%ASiR-V.11 groups of monochromatic energy images(40-140keV,with 10keV interval)and conventional 120kVp-like images were reconstructed.The objective image quality including CT value,the CT value standard deviation(SD),and contrast to noise ratio(CNR)of The portal vein(PV),superior mesenteric vein(SMV),superior mesenteric artery(SMA)and abdominal aorta(AA)on each low keV images and conventional 120kVp-like images were measured.The subjective image quality of them were scored.The difference of image quality were test with one-way analysis of variance(ANOVA)and Wilcoxon rank sum test(α=0.05).Results:The score of 60keV images(4.64±0.68)is higher than that of conventional 120kVp-like images(3.93±0.60)(P<0.05).On the 40keV images,the contrast between blood vessel and liver parenchyma is increased,the SD is significantly increased,the overall image was rough,and the observation was significantly limited.The enhancement effect of monochromatic energy images becomes worse gradually when monochromatic energy exceeds 60keV.The CT value,CNR and SD of each vessel on 40-70keV monochromatic energy images are decreased when the monochromatic energy is increasing gradually.The CT value and CNR on 40-70keV monochromatic energy images are higher than that of conventional 120kVp-like images(P<0.05).The enhancement effect is superior to that of conventional 120kVp-like images.There is the difference between the CNR of the system default optimal monochromatic energy 40keV and 60keV(P<0.05).The SD of low keV monochromatic energy is higher than that of conventional 120kVp-like images.The image quality of 50-70keV monochromatic energy image are accepted making diagnosis.Conclusion:The revolution CT combined with new ASiR-V is availabl
