摘　　要：目的探讨音乐疗法联合安抚奶嘴对先天性心脏病术后脱离呼吸机患儿镇静效果的影响。方法将160例先天性心脏病术后撤离呼吸机的患儿随机分为观察组和对照组各80例,对照组给予先天性心脏病术后常规护理,观察组在对照组的基础上给予音乐疗法联合安抚奶嘴;干预后,比较2组情绪状态,是否出现屏气、喉头水肿及心力衰竭等并发症,并比较2组干预前后心率、收缩压、呼吸频率、血浆乳酸值、血氧饱和度等指标的变化。结果干预后,观察组心率、收缩压、呼吸频率和心力衰竭发生率均低于对照组(t=6.041,P<0.001;t=10.126,P<0.001;t=2.777,P=0.006;χ2=4.444,P=0.035),患儿安静率和血氧饱和度均高于对照组(χ2=7.156,P=0.008;t=2.467,P=0.015)。结论音乐疗法联合安抚奶嘴的干预方法可改善先天性心脏病患儿术后的循环指标,减少患儿哭闹,减轻患儿激惹状态,使患儿保持安静,减少心力衰竭的发生。Objective To explore the sedative effect of music therapy combined with pacifier on postoperative children with congenital heart disease(CHD)after weaning from mechanical ventilation.Methods A total of 160 CHD children weaning from mechanical ventilation were randomly divided into an observation group and a control group,each of 80.Both groups were given routine postoperative nursing,while the observation group was additionally provided with music therapy and pacifier.The emotional state of the two groups was compared,and the complications such as breath holding,laryngeal edema and heart failure were observed.The heart rate,systolic blood pressure,respiratory rate,plasma lactic acid value and blood oxygen saturation of both groups were recorded before and after the intervention.Results After the intervention,the heart rate,systolic blood pressure,respiratory rate and heart failure of the observation group were significantly lower than that of the control group(t=6.041,P<0.001;t=10.126,P<0.001;t=2.777,P=0.006;χ2=4.444,P=0.035),while the quietness rate and blood oxygen saturation were significantly higher than that of the control group(χ2=7.156,P=0.008;t=2.467,P=0.015).Conclusion The music therapy combined with pacifier can improve the postoperative blood circulation of CHD children,reduce their crying and agitation,as well as the occurrence of heart failure.
