摘  要:探讨核磁共振在大腿后侧肌肉群慢性运动损伤中的成像表现及诊断价值.方法:选取2019年4月-2020年6月在我院外科或骨科就诊的30例大腿后侧肌肉群损伤患者作为研究对象,于损伤7天内进行核磁共振(MRI)及超声检查,观察MRI及超声的成像表现,分析大腿后侧肌群损伤后的MRI成像特征,并与超声检查进行比较.结果:30例大腿后侧肌肉群损伤患者中共累计损伤肌肉52块,其中单一损伤15例,联合损伤15例;一级损伤36块,二级损伤11块,三级损伤5块.MRI成像显示一级损伤肌腱未见异常,二级损伤肌腱部分断裂,三级损伤肌腱断裂分离.MRI征象显示一级损伤患者均出现肌间束渗出,二级损伤患者除肌间束渗出外还出现筋膜渗出与肌断层积液,三级损伤患者4种征象均出现.与超声检查相比,MRI检查在肌肉撕裂方面检出率更高(P<0.05);在轻度损伤方面超声优于MRI(P<0.05).结论:对于大腿后侧肌群损伤而言,MRI与超声均有较好的检出率,但MRI可对于损伤的部位、范围、程度、类型有更清晰的反映,具有较高的诊断价值,适合临床中应用与选择.Objective:To investigate the imaging findings and diagnostic value of NUCLEAR magnetic resonance imaging(MRI)in chronic motor injury of posterior thigh muscles.Methods:April 2019-June 2020 in our hospital surgery or orthopedic clinic of 30 cases back thigh muscle injury patients as the research object,the damage within 7 days of nuclear magnetic resonance(MRI)and ultrasonic examination,to observe MRI and ulrasound imaging performance,analysis of back thigh muscle injury after the MRI imaging features,and comparing with ultrasound.Results:A total of 52 muscles were injured in 30 patients with posterior thigh muscle group injury,including 15 patients with single injury and 15 patients with combined injury.There were 36 first-grade injuries,1l second-grade injuries and 5 third-grade injuries.MRI imaging showed no abnormality of the tendon in the first-degree injury,partial fracture of the tendon in the second-degree injury,and fracture separation of the tendon in the third-degree injury.MRI signs showed that intramuscular tract exudation occurred in all patients with first-degree injury;in addition to intramuscular tract exudation in patients with second-degree injury,fascial exudation and muscular sectional effusion also occurred,and all four signs appeared in patients with third-degree injury.Compared with ultrasound,MRI showed a higher detection rate of muscle tear(P<O.05).Ultrasound was superior to MRI in mild injury(P<0.05).Conclusion:MRI and ultrasound have a good detection rate for the injury of the posterior thigh muscle group,but MRI can clearly reflect the site,scope,degree and type of the injury,which has a high diagnostic value and is suitable for clinical application and selection.
