止嗽散加味治疗变应性咳嗽临床疗效及对血清总免疫球蛋白E、嗜酸性粒细胞的影响

摘　　要：目的:基于“风燥犯肺入络”理论,研究止嗽散加味治疗变应性咳嗽患者的临床疗效及机制。方法:选取符合纳入标准的变应性咳嗽患者90例,随机分为止嗽散加味治疗组和对照组各45例,对照组给予沙美特罗替卡松粉吸入剂联合孟鲁司特钠,治疗组给予止嗽散加味治疗,治疗2周后观察2组患者咳嗽症状积分、血清总免疫球蛋白E(immunoglobulin E,IgE)和嗜酸性粒细胞(eosinophils,EOS)并评定临床疗效,治疗期间观察药物不良反应,停药后随诊4周观察咳嗽复发率。结果:治疗后2组患者咳嗽症状积分、血清总IgE和EOS均较治疗前下降,差异有统计学意义(P<0.05,P<0.01);与对照组比较,治疗组下降更明显(P<0.05,P<0.01),显控率和总有效率均高于对照组,不良反应发生率和复发率均低于对照组,差异有统计学意义(P<0.05)。结论:止嗽散加味治疗变应性咳嗽疗效显著,可降低患者血清总IgE和EOS,具有抗变态反应作用,且不良反应和复发率低,值得进一步推广研究。Objective:To study the clinical effect and mechanism of modified Zhisou powder on atopic cough based on the theory of“wind dryness invading lung and its collaterals”.Methods:90 patients with atopic cough were randomly divided into treatment group and control group,45 cases in each group.The control group was given salmeterol and fluticasone powder inhalation combined with montelukast sodium,and the treatment group was given modified Zhisou powder.After 2 weeks of treatment,the cough symptom score,serum total IgE and EOS were observed and detected,while the clinical effect was evaluated in the two groups.The adverse drug reactions were observed during the treatment,and the cough recurrence rate was observed after 4 weeks of follow-up.Results:After treatment,the cough symptom score,serum total IgE and EOS of the two groups decreased significantly compared with those before treatment(P<0.05,P<0.01).Compared with control group,the change was more obvious in the treatment group(P<0.05,P<0.01).The significant control rate and total effective rate of the treatment group were higher than those of the control group,while the incidence of adverse reactions and recurrence rate were lower than those of the control group,the difference between the two groups was statistically significant(P<0.05).Conclusion:Modified Zhisou powder has significant effect on atopic cough,with low adverse reactions and recurrence rate.It can reduce serum total IgE and EOS,and has the effect of antiallergic reaction,which is worth further popularizing and investigation.
