超声引导下针刀松解术治疗屈指肌腱腱鞘炎的效果观察及超声影像前后比对研究

摘　　要：目的:探讨超声引导下针刀松解术治疗屈指肌腱腱鞘炎的效果及超声影像前后对比。方法:选取2020年2月-2021年3月深圳市宝安区中医院收治的120例屈指肌腱腱鞘炎患者进行回顾性分析,将其根据治疗方案分为A、B、C组,每组40例。A组行超声引导下针刀松解术治疗,B组行非直视下针刀治疗,C组行超声引导下针刺治疗,比较三组治疗结果。结果:三组术前数字疼痛评分法(NRS)评分比较,差异无统计学意义(P>0.05),A组术后1周和术后1、2、3个月NRS评分均较B组、C组低(P<0.05);A组、B组总有效率分别为97.50%、95.00%,较C组高(P<0.05)。术前三组捡小物体、翻纸牌、模拟进食、移动罐头各测试结果比较,差异无统计学意义(P>0.05),术后,A组Jebsen手功能测试结果与B组、C组比较,差异有统计学意义(P<0.05)。三组术前超声影像比较,差异无统计学意义(P>0.05),A组术后超声影像与B组、C组比较,差异有统计学意义(P<0.05)。三组均能治疗腱鞘炎,A组治疗次数、恢复期与B组比较,差异有统计学意义(P<0.05),C组疗效较差,无法恢复。结论:超声引导下针刀松解术治疗屈指肌腱腱鞘炎效果确切,可减轻术后疼痛程度,改善患者手功能及超声影像,值得应用。Objective:To investigate the effect of ultrasound-guided needle knife release in the treatment of flexor tendon tenosynovitis and the comparison of ultrasound images before and after.Method:A total of 120 patients with flexor tendon tenosynovitis admitted to Shenzhen Bao’an District Hospital of Traditional Chinese Medicine from February 2020 to March 2021 were selected for retrospective analysis,they were divided into groups A,B and C according to the treatment scheme,with 40 cases in each group.Group A was treated with needle knife release under ultrasound-guided,group B was treated with needle knife under non direct vision,and group C was treated with needle knife under ultrasound-guided.The treatment results of the three groups were compared.Result:There was no significant difference in NRS scores among the three groups before operation (P>0.05),NRS scores of group A were lower than those of group B and group C at 1 week and 1,2 and 3 months after operation (P<0.05);the total effective rates of group A and group B were 97.50% and 95.00% respectively,which were higher than that of group C (P<0.05).There were no significant differences in the test results of picking up small objects,flipping cards,simulated eating and moving cans among the three groups before operation (P>0.05),after operation,the Jebsen hand function test results in group A were significantly different from those in group B and group C (P<0.05).There was no significant difference in preoperative ultrasound images among the three groups (P>0.05),there was significant difference in postoperative ultrasound images between group A,group B and group C (P<0.05).The three groups were able to treat tenosynovitis,the treatment times and recovery period of group A were significantly different from those of group B (P<0.05),the curative effect of group C was poor and could not recover.Conclusion:Ultrasound-guided acupotomy for the treatment of flexor tendon tenosynovitis is effective,which can reduce postoperative pain,improve hand function and ultrasou
