经皮内固定与开放式内固定对多椎体胸腰椎骨折患者的疗效比较

摘　　要：目的:比较经皮内固定与开放式内固定对多椎体胸腰椎骨折患者的疗效。方法:选取2019年3月-2021年3月在如皋市中医院骨科治疗的85例多椎体胸腰椎骨折患者作为研究对象,采用随机数字表法分为对照组42例(开放式内固定)和研究组43例(经皮内固定)。比较两组置钉准确度、手术相关指标、炎症因子、肌酸激酶活性、治疗满意度。结果:研究组置钉准确度略高于对照组,但差异无统计学意义(P>0.05)。研究组住院时间、术中出血量均优于对照组(P<0.05);两组手术时间比较,差异无统计学意义(P>0.05)。研究组术后CRP、IL-6水平均低于对照组(P<0.05)。研究组术后1、2 d肌酸激酶活性均低于对照组(P<0.05)。研究组治疗满意度为97.7%,高于对照组的85.7%(P<0.05)。结论:经皮内固定治疗多椎体胸腰椎骨折患者,可减少术中出血量和缩短住院时间,抑制炎症因子的产生,降低肌酸激酶活性,提升患者的治疗满意度。Objective:To compare the efficacy of percutaneous internal fixation and open internal fixation in patients with multi vertebral thoracolumbar fractures.Method:A total of 85 patients with multi vertebral thoracolumbar fractures who treated in the Department of Orthopedics of Rugao Hospital of Traditional Chinese Medicine from March 2019 to March 2021 were selected as the research objects.They were randomly divided into 42 cases in the control group (open internal fixation) and 43 cases in the study group (percutaneous internal fixation) according to the random number table method.The accuracy of nail placement,operation related indexes,inflammatory factors,creatine kinase activity and curative satisfaction of two groups were compared.Result:The accuracy of nail placement in the study group was slightly higher than that in the control group,but the difference was not statistically significant (P>0.05).The hospitalization time and intraoperative bleeding volume of the study group were better than those of the control group (P<0.05);there was no significant difference in the operation time between two groups (P>0.05).The CRP and IL-6 levels in the study group after operation were lower than those in the control group (P<0.05).The creatine kinase activity in the study group at 1 and 2 d after operation were lower than those in the control group (P<0.05).The treatment satisfaction of the study group was 97.7%,which was higher than 85.7% of the control group (P<0.05).Conclusion:Percutaneous internal fixation for patients with multi vertebral thoracolumbar fractures can reduce the amount of intraoperative bleeding and hospital stay,inhibit the production of inflammatory factors,reduce the activity of creatine kinase and improve the treatment satisfaction of patients.
