补肾调冲汤治疗脾肾阳虚型早发性卵巢功能不全效果观察

摘　　要：目的:探究脾肾阳虚型早发性卵巢功能不全(premature ovarian insufficiency,POI)应用补肾调冲汤治疗的效果。方法:将2019年10月-2020年9月福建中医药大学附属人民医院收治的65例脾肾阳虚型POI患者纳入研究,根据随机排列法进行分组,包括对照组32例,采取常规西医治疗方案;治疗组33例,加用补肾调冲汤治疗。观察两组用药前(首次就诊时)及用药后(治疗3个疗程后)的症状积分、性激素水平、子宫内膜厚度变化及不良反应。结果:用药后,两组改良Kupperman积分均低于用药前,且治疗组低于对照组(P<0.05)。用药后,两组血清卵泡刺激素(follicle stimulating hormone,FSH)、黄体生成素(luteinizing hormone,LH)水平均低于用药前,雌二醇(estrogen,E)水平高于用药前(P<0.05);且治疗组FSH、LH水平低于对照组,E水平高于对照组(P<0.05)。用药后,两组子宫内膜厚度均大于用药前,且治疗组大于对照组(P<0.05)。两组用药期间均无不良反应及血常规、肝肾功能异常。结论:补肾调冲汤可有效控制患者的症状,并对机体的激素分泌及子宫内膜厚度产生调节作用,且无明显不良反应,应用于脾肾阳虚型POI中安全有效。Objective:To explore the effect of Bushen Tiaochong Decoction in the treatment of premature ovarian insufficiency(POI)with spleen and kidney Yang deficiency type.Method:A total of 65 patients with POI of spleen and kidney Yang deficiency type admitted to the People’s Hospital Affiliated to Fujian University of Traditional Chinese Medicine from October 2019 to September 2020 were included in the study.They were divided into groups according to random arrangement,including 32 cases in the control group,who were treated with conventional western medicine,33 cases in the treatment group,added Bushen Tiaochong Decoction treatment.The symptom scores,sex hormone levels,endometrial thickness and adverse reactions were observed before medication(at the first visit)and after medication(after 3 courses of treatment).Result:After medication,the improved Kupperman score of the two groups were lower than those before treatment,and the treatment group was lower than that in the control group(P<0.05).After medication,the levels of serum follicle stimulating hormone(FSH),luteinizing hormone(LH)in both groups were lower than those before treatment,and the level of estradiol(E)was higher than that before treatment(P<0.05),and the levels of FSH and LH in the treatment group were lower than those in the control group,and the level of Ewas higher than that in the control group(P<0.05).After medication,the endometrial thickness of the two groups were greater than that before treatment,and that in the treatment group was greater than that in the control group(P<0.05).There were no adverse reactions,blood routine,liver and kidney function abnormalities in both groups during the treatment period.Conclusion:Bushen Tiaochong Decoction can effectively control the symptoms of patients,and regulate the secretion of hormones and endometrial thickness in the body without obvious adverse reactions.It is safe and effective in POI with spleen and kidney Yang deficiency.
