摘 要:目的:研究不同翻身时间俯卧位通气对急性呼吸窘迫综合征(ARDS)患者的影响。方法:选取2020年1月-2021年12月福州市第二医院ICU收治的58例ARDS患者作为研究对象。根据随机分组将其分为试验组与对照组,每组29例。两组均给予俯卧位通气,对照组俯卧3 h→平卧1 h→俯卧3 h,翻身2次,试验组俯卧2 h→平卧1 h→俯卧2 h→平卧1 h→俯卧2 h。比较两组并发症、俯卧位通气治疗前后的临床指标、氧合指数及氧合良好率。结果:两组并发症发生率比较差异无统计学意义(P>0.05)。治疗后,两组心率(HR)、气道平均压(P_(mean))、潮气量(VT)、平均动脉压(MAP)、氧分压(PaO_(2))比较差异均无统计学意义(P>0.05)。治疗后,两组氧合指数均较治疗前升高,但两组氧合指数及氧合良好率比较差异均无统计学意义(P>0.05)。结论:不同翻身时间俯卧位通气并不会影响ARDS患者氧合指数和并发症、临床体征。Objective:To study the effect of prone position ventilation with different turning over time on patients with acute respiratory distress syndrome(ARDS).Method:Fifty-eight ARDS patients admitted to the ICU of Fuzhou Second Hospital from January 2020 to December 2021 were selected as the study subjects.According to the randomization grouping,they were divided into the test group and the control group,with 29 cases in each group.Both groups were given prone position ventilation.The control group was prone for 3 h→prostrate for 1 h→prone for 3 h,and turned over twice.The test group was prone for 2 h→prostrate for 1 h→prone for 2 h→prostrate for 1 h→prone for 2 h.The complications,clinical indexes,oxygenation index and good oxygenation rate before and after treatment of prone position ventilation were compared between the two groups.Result:There was no significant difference in the incidence of complications between the two groups(P>0.05).After treatment,there were no significant differences in heart rate(HR),mean airway pressure(P_(mean)),volume of tidal(VT),mean arterial pressure(MAP)and partial arterial partial pressure of oxygen(PaO_(2))between the two groups(P>0.05).After treatment,the oxygenation index of the two groups was higher than that before treatment,but there were no significant differences in the oxygenation index and good oxygenation rate between the two groups(P>0.05).Conclusion:Prone position ventilation with different turning over time do not affect the oxygenation index,complications and clinical signs of ARDS patients.
