摘 要:目的:探究唑来膦酸对老年股骨骨折髋关节置换患者的影响。方法:将三明市第二医院2019年6月-2021年2月收治的86例老年股骨骨折髋关节置换患者按照随机数字表法分为对照组和观察组,各43例。对照组进行常规术后治疗,观察组则在对照组的基础上加用唑来膦酸。比较治疗后6、12个月两组的功能状态(Harris评分总优良率),比较治疗前及治疗后6、12个月两组的骨密度水平及骨转换指标[血清Ⅰ型胶原氨基末端肽(NTx)、Ⅰ型胶原羧基末端肽(CTx)];比较治疗前及治疗后4、8、12周两组的疼痛情况[视觉模拟评分法(VAS)评分],比较治疗后两组的不良情况发生率。结果:治疗后6、12个月观察组的总优良率显著高于对照组,差异有统计学意义(P<0.05)。治疗前两组的VAS评分、骨密度水平及骨转换指标比较,差异无统计学意义(P>0.05),治疗后4、8、12周观察组的VAS评分显著低于对照组,差异有统计学意义(P<0.05);治疗后6、12个月观察组的骨密度水平显著高于对照组,骨转换指标显著低于对照组,差异有统计学意义(P<0.05)。治疗后观察组的不良情况发生率显著低于对照组,差异有统计学意义(P<0.05)。结论:唑来膦酸可显著改善老年股骨骨折髋关节置换患者的骨转换及功能状态,临床应用价值较高。Objective:To investigate the influence of Zoledronic Acid in elderly patients with hip replacement for femoral fracture.Method:A total of 86 elderly patients with hip replacement for femoral fracture treated in Sanming Second Hospital from June 2019 to February 2021 were divided into control group and observation group according to the random number table,with 43 cases in each group.The control group was treated with routine postoperative treatment,the observation group was treated with Zoledronic Acid on the basis of control group.The function states(total excellent and good rates of Harris score)of the two groups were compared at 6 and 12 months after the treatment.Bone mineral density and bone turnover indexes[serum typeⅠcollagen amino-terminal peptide(NTx)and typeⅠcollagen carboxy-terminal peptide(CTx)]were compared between the two groups before treatment and 6 and 12 months after treatment.The pain[visual analog scale(VAS)score]of the two groups were compared before treatment and 4,8 and 12 weeks after treatment.The incidence of adverse events was compared between the two groups after treatment.Result:The total excellent and good rates in the observation group at 6 and 12 months after the treatment were significantly higher than those in the control group,the difference was statistically significant(P<0.05).There were no significant differences in VAS score,bone mineral density level and bone turnover indexes between the two groups before treatment(P>0.05).At 4,8 and 12 weeks after treatment,the VAS score in the observation group were significantly lower than those in the control group(P<0.05).At 6 and 12 months after treatment,the bone mineral density levels in the observation group were significantly higher than those in the control group,the bone turnover indexes were significantly lower than those in the control group,the differences were statistically significant(P<0.05).After treatment,the incidence of adverse events in the observation group was significantly lower than that in the control group(P<
