摘  要:目的:探究不同维生素K_(1)预防性给药剂量对低出生体重早产儿的影响。方法:选取佛山市三水区人民医院新生儿科2020年7月-2021年12月120例低出生体重早产儿作为研究对象。根据随机数表法将其分为观察组(n=60)和对照组(n=60)。两组均在出生时抽脐带血2 ml检测血浆维生素K(VK)水平。对照组在出生30 min内肌注维生素K_(1)0.5 mg,观察组于相同时间段肌注维生素K_(1)1 mg。比较两组用药后30 d VK缺乏性出血(VKDB)发生情况,用药前、用药后24 h血浆VK水平,用药前及用药后14 d PT、APTT及FIB指标、VK依赖因子活性。结果:用药后30 d,两组VKDB发生率比较差异无统计学意义(P>0.05)。用药后24 h,观察组VK水平显著高于对照组(P<0.05)。用药后14 d,两组PT、APTT比用药前显著缩短(P<0.05),FIB水平比用药前显著升高(P<0.05),两组PT、APTT及FIB比较差异均无统计学意义(P>0.05)。用药后14 d,两组Ⅱ、Ⅶ、Ⅸ、Ⅹ因子活性均高于用药前,两组Ⅱ、Ⅶ、Ⅸ、Ⅹ因子活性比较差异均无统计学意义(P>0.05)。结论:出生30 min内肌注VK_(1)可有效地提升血浆VK,然而不同剂量的VK_(1)治疗效果相同,推荐使用0.5 mg剂量即可达到比较理想的效果。Objective:To investigate the effects of different doses of vitamin K_(1) prophylactic administration on premature infants with low birth weight.Method:A total of 120 cases of low birth weight premature infants in the Department of Neonatology of Sanshui District People’s Hospital of Foshan from July 2020 to December 2021 were selected as the research objects.They were divided into observation group(n=60)and control group(n=60)according to random number table method.At birth,2 ml umbilical cord blood was drawn from both groups to detect plasma vitamin K(VK)levels.The control group was intramuscularly injected with Vitamin K_(1)0.5 mg within 30 minutes of birth,and the observation group was intramuscularly injected with Vitamin K_(1)1 mg at the same time.The occurrence condition of VK deficiency bleeding(VKDB)30 d after medication,plasma VK level before medication and 24 h after medication,PT,APTT and FIB indexes and VK dependent factor activity before and 14 d after medication were compared between the two groups.Result:Thirty days after medication,there was no significant difference in the incidence of VKDB between the two groups(P>0.05).24 h after medication,the level of VK in the observation group was significantly higher than that in the control group(P<0.05);14 d after medication,PT,APTT in the two groups were significantly shorter than those before medication(P<0.05),and FIB level was significantly higher than that before medication(P<0.05),there were no significant differences in PT,APTT and FIB between the two groups(P>0.05);14 d after medication,the activities of factorsⅡ,Ⅶ,ⅨandⅩin the two groups were higher than those before medication,and there were no significant differences in the activities of factorsⅡ,Ⅶ,ⅨandⅩbetween the two groups(P>0.05).Conclusion:Intramuscular injection of VK_(1) within 30 minutes of birth can effectively increase plasma VK.However,different doses of VK_(1) have the same therapeutic effect,and it is recommended to use 0.5 mg dose to achieve the ideal effect.
