摘 要:目的:分析糖尿病周围神经病变(DPN)患者神经电生理变化情况及其与患者病情的关系。方法:以2019年1月-2021年1月泸县人民医院95例DPN患者为对象(观察组),以95例健康人作为对照组。检测两组神经电生理指标,分析其与DPN患者病情的关系。结果:观察组运动神经传导速度、波幅低于对照组,远端潜伏期高于对照组(P<0.05)。观察组皮肤交感反应(SSR)异常、上肢躯体感觉诱发电位(SEP)异常、下肢SEP异常率分别为93.69%、73.68%、76.84%,高于对照组(4.21%、7.37%、8.42%)(P<0.05)。运动神经传导速度、波幅在Ⅰ级、Ⅱ级、Ⅲ级、Ⅳ级中依次降低,远端潜伏期依次升高(P<0.05)。上肢SEP异常异常率在Ⅰ级、Ⅱ级、Ⅲ级、Ⅳ级中差异有统计学意义(P<0.05)。Pearson相关性分析显示DPN患者病情分级与运动神经传导速度、波幅呈负相关,与远端潜伏期呈正相关(P<0.05);Spearman相关性分析显示DPN病情分级与SSR异常、上肢SEP异常、下肢SEP异常正相关(P<0.05)。结论:神经电生理指标可用于DPN的早期诊断并且可评估患者的病情。Objective:To analyze the electrophysiological changes in patients with diabetic peripheral neuropathy(DPN)and their relationship with the patients’illness condition.Method:A total of 95 patients with DPN from January 2019 to January 2021 in Lu County People’s Hospital were selected as the observation group and 95 healthy persons were selected as the control group.The neuroelectrophysiological indexes of the two groups were detected and their relationship with the illness condition of patients with DPN was analyzed.Result:The motor nerve conduction velocity and amplitude in the observation group were lower than those in the control group,and the distal latency was higher than that in the control group(P<0.05).The abnormal rates of skin sympathetic response(SSR),upper limb somatosensory evoked potential(SEP)and lower limb SEP in the observation group were 93.69%,73.68%and 76.84%respectively,which were higher than 4.21%,7.37%and 8.42%in the control group(P<0.05).Motor nerve conduction velocity and amplitude decreased in gradeⅠ,Ⅱ,ⅢandⅣ,and the distal latency increased in turn(P<0.05).The upper limb SEP in gradeⅠ,Ⅱ,ⅢandⅣhad significant differences(P<0.05).Pearson correlation analysis showed that the disease grade of DPN patients was negatively correlated with motor nerve conduction velocity and amplitude,and positively correlated with distal latency(P<0.05);Spearman correlation analysis showed that DPN disease grade was positively correlated with SSR abnormality,upper limb SEP abnormality and lower limb SEP abnormality(P<0.05).Conclusion:Neuroelectrophysiological indexes can be used in the early diagnosis of DPN and can evaluate the illness condition of patients.
