美式整脊联合传统推拿疗法治疗神经根型颈椎病的临床效果

摘　　要：目的:探讨美式整脊联合传统推拿疗法治疗神经根型颈椎病(cervical spondylotic radiculopathy,CSR)的临床效果。方法:选取2020年1月-2022年1月中南大学湘雅医院康复科门诊收治的42例CSR患者。随机将其分为对照组和试验组,各21例。对照组采用传统推拿疗法,试验组采用美式整脊联合传统推拿疗法。比较两组临床疗效,治疗前后颈椎疼痛度、颈椎活动度、病情严重程度、生活质量、抑郁情况及疾病复发。结果:试验组治疗总有效率高于对照组(P<0.05)。治疗后,试验组VAS评分低于对照组,颈椎向健侧旋转活动度、颈椎向健侧侧屈活动度均大于对照组(P<0.05)。治疗后,试验组主观症状、生活、工作和社会适应能力、临床体征评分均高于对照组(P<0.05)。治疗后,试验组生理健康(PCS)、心理健康(MCS)评分均高于对照组,汉密尔顿抑郁量表(HAMD)评分低于对照组(P<0.05)。两组复发率比较差异无统计学意义(P>0.05)。结论:美式整脊联合传统推拿疗法可改善CSR患者疼痛程度、颈椎活动度、颈椎病严重程度、生活质量和不良心理状态,最终提高总有效率。Objective:To investigate the clinical effect of American chiropractic combined with traditional massage therapy in the treatment of cervical spondylotic radiculopathy(CSR).Method:From January 2020 to January 2022,42 patients with CSR were selected from the Outpatient of Rehabilitation Department of Xiangya Hospital of Central South University.They were randomly divided into control group and experimental group,with 21 cases in each group.The control group was treated with traditional massage therapy,and the experimental group was treated with American chiropractic combined with traditional massage therapy.The clinical effects,cervical pain degree,cervical mobility,disease severity,quality of life,depression condition before and after treatment and disease recurrence of the two groups were compared.Result:The treatment total effective rate of the experimental group was higher than that of the control group(P<0.05).After treatment,the VAS score of the experimental group was lower than that of the control group,and the rotation and flexion mobility of the cervical spine to the healthy side were greater than those of the control group(P<0.05).After treatment,the scores of subjective symptoms,life,work and social adaptability,and clinical signs in the experimental group were higher than those in the control group(P<0.05).After treatment,the scores of physical component summary scale(PCS)and mental component summary scale(MCS)in the experimental group were higher than those in the control group,and the score of Hamilton depression scale(HAMD)was lower than that in the control group(P<0.05).There was no significant difference in the recurrence rate between the two groups(P>0.05).Conclusion:American chiropractic combined with traditional massage therapy can improve the degree of pain,cervical mobility,severity of cervical spondylosis,quality of life and poor psychological state of patients with CSR,and ultimately improve the total effective rate.
