摘　　要：目的:探究社区干预措施对基层医院中慢性房颤患者抗凝治疗的效果。方法:选2020年5月-2021年5月经中山市东凤人民医院门诊、急诊或心内科确诊的100例慢性心房颤动患者为入选对象,采用临床试验中央随机系统将其随机分为试验组和对照组,各50例,试验组予以社区干预联合华法林抗凝治疗方案,对照组仅予以单纯华法林抗凝治疗,在治疗12个月后评估两组服药依从性、国际标准化比率(INR)监测情况、抗凝达标率及并发症发生率。结果:与对照组相比,试验组治疗后宣教掌握情况、华法林抗凝治疗依从性、定期检测INR率及抗凝达标率均优于对照组,同时并发症发生率低于对照组,差异均有统计学意义(P<0.05)。结论:在急诊专科医生组成的抗栓团队指导下,采取社区干预模式可明显提高基层医院中慢性房颤患者抗凝治疗的依从性、INR达标率、抗凝达标率及减少了患者出血、血栓、心源性卒中等意外情况的发生率,提高了基层医院慢性房颤防治水平。Objective:To explore the effect of community intervention measures on anticoagulation treatment of patients with chronic atrial fibrillation in primary hospital.Method:From May 2020 to May 2021,100 patients with chronic atrial fibrillation who were diagnosed in Outpatient Department,Emergency Department or Cardiology Department of Dongfeng People’s Hospital of Zhongshan City were selected as the selected subjects,and they were randomly assigned the experimental group and the control group by the central randomization system of clinical trials,50 cases in each group.The experimental group was given community intervention combined with Warfarin anticoagulation treatment plan,and the control group was given only Warfarin anticoagulation therapy.After 12 months of treatment,the patients’ medication compliance,INR monitoring,anticoagulation compliance rate and complication rate of two groups were evaluated.Result:Compared with the control group,the propaganda and education mastery,compliance with Warfarin anticoagulation therapy,regular INR detection rate and anticoagulant compliance rate of the experimental group after treatment were significantly better than those of the control group,and the incidence of complications was significantly lower than the control group,the differences were statistically significant (P<0.05).Conclusion:Under the guidance of an antithrombotic team composed of emergency specialists,the adoption of a community intervention model can significantly improve the compliance of anticoagulant treatment,INR compliance rate,anticoagulation compliance rate and reduce of the incidence of accidents such as bleeding,thrombosis,and cardiogenic stroke in patients with chronic atrial fibrillation in primary hospitals,improve the prevention and treatment of chronic atrial fibrillation in primary hospitals.
