颈椎后路单、双开门椎管扩大成形术治疗多节段脊髓型颈椎病的早期临床疗效比较

摘　　要：目的:分析比较颈椎后路单、双开门椎管扩大成形术治疗多节段脊髓型颈椎病的临床疗效。方法:选取35例多节段脊髓型颈椎病患者为研究对象,分为单开门组(18例,行颈椎后路单开门椎管扩大成形术)与双开门组(17例,行颈椎后路双开门椎管扩大成形术)。对两组患者术前及术后的日本矫形外科协会评分(JOA)及Hirabayashi改善率、颈椎曲度、脊髓漂移距离进行比较。结果:两组患者的临床疗效、颈椎曲度、脊髓漂移距离差异无统计学意义(P>0.05),单开门组出血量多于双开门组(P<0.05),且单开门组手术时间较双开门短(P<0.05)。结论:在多节段脊髓型颈椎病中,单开门椎管扩大成形术与双开门扩大椎管成形术在临床疗效、颈椎曲度改变、脊髓漂移距离等方面差异无统计学意义(P>0.05),其中单开门椎管扩大成形术出血量较多,但手术时间较短。临床医学可根据患者不同适应证开展治疗,以期更好的修复效果。Objective To analyze and compare the clinical effect of single and double open-door posterior cervical expansive canal plasty in the treatment of multilevel cervical spondylotic myelopathy.Method 35 patients with multilevel cervical spondylotic myelopathy were selected as research object, they were divided into the single-door group(18 patients underwent posterior single-door Laminoplasty) and the double-door group(17 patients underwent posterior double-door Laminoplasty).Preoperative and postoperative JOA score, Hirabayashi recovery rate, cervical curvature and spinal cord drift distance were compared between the two groups.Results There were no significant differences in clinical effects, cervical curvature and spinal cord drift distance between the two groups(P>0.05). The blood loss in the single-door group was more than that in the double-door group(P<0.05), and the operation time in the single-door group was shorter than that in the double-door group(P<0.05).Conclusion In multilevel cervical spondylotic myelopathy, there was no significant difference in clinical efficacy, cervical curvature change and spinal cord drift distance between open-door dilated spondyloplasty and double-door dilated spondyloplasty(P>0.05), among which, open-door dilated spondyloplasty had more bleeding but shorter operation time.
