Ⅲ度房室传导阻滞起搏器植入术后囊袋感染影响因素分析

摘　　要：目的探讨Ⅲ度房室传导阻滞起搏器植入术后囊袋感染的独立危险因素。方法选择2018年5月-2021年5月收治的行起搏器植入术的Ⅲ度房室传导阻滞100例,并在术后随访1年,将发生囊袋感染的15例设为感染组,将未发生感染的85例设为非感染组,分析起搏器植入术后囊袋感染发生的影响因素。结果合并糖尿病、起搏器植入时间≥3 h、起搏器植入次数≥2次、发生囊袋血肿、切口愈合不良、发生贫血、术前未预防性使用抗菌药物、术前血清蛋白及血红蛋白水平低为Ⅲ度房室传导阻滞起搏器植入术后囊袋感染的影响因素(P<0.05,P<0.01)。结论合并糖尿病、起搏器植入时间与次数、囊袋血肿及切口愈合情况、术前预防性使用抗菌药物情况及术前机体营养水平等因素与Ⅲ度房室传导阻滞起搏器植入术后囊袋感染的发生具有相关性,临床可根据上述影响因素制订针对性干预措施,预防感染,改善预后。Objective To explore the independent risk factors of pocket infection after pacemaker implantation in patients with third-degree atrioventricular block.Methods We selected 100 patients with third-degree atrioventricular block who underwent pacemaker implantation from May 2018 to May 2021,and they were followed up for 1 year after surgery.Of them,15 patients with pocket infection were set as the infection group,and 85 cases without infection were set as non-infection group.The influencing factors of pocket infection after pacemaker implantation were analyzed.Results Diabetes mellitus,pacemaker implantation time≥3 h,pacemaker implantation times≥2 times,pocket hematoma,poor incision healing,anemia,no prophylactic use of antibiotics before surgery,and low levels of preoperative serum protein and hemoglobin levels were the influencing factors for the occurrence of pocket infection after pacemaker implantation in patients with third-degree atrioventricular block(P<0.05,P<0.01).Conclusion Diabetes mellitus,time and frequency of pacemaker implantation,pocket hematoma and wound healing,preoperative prophylactic use of antibiotics,and preoperative nutritional level are associated with third-degree atrioventricular block after pacemaker implantation.Targeted interventions can be formulated according to the above influencing factors to prevent infection and improve prognosis.
