摘 要:目的分析膝关节骨性关节炎(KOA)患者中性粒细胞与淋巴细胞比值(NLR)、骨桥素(OPN)、乏氧诱导因子-1α(HIF-1α)水平与膝关节功能、肌力的相关性。方法选取2020年4月-2021年5月收治的KOA 82例作为KOA组,另选取同期健康体检者40例作为健康对照组。比较2组及不同病情程度KOA患者NLR、OPN、HIF-1α水平及膝关节功能[骨性关节炎自评量表(WOMAC)评分]、肌力[峰力矩(PT)、腘绳肌与股四头肌峰力矩比值(H/Q)],采用Spearman相关性分析探讨KOA患者NLR、OPN、HIF-1α水平与膝关节功能、肌力的相关性。结果KOA组中性粒细胞、NLR、OPN、HIF-1α水平和WOMAC评分、H/Q均高于健康对照组,伸膝肌群和屈膝肌群PT低于健康对照组(P<0.01)。KOA患者随着病情逐渐加重,NLR、OPN、HIF-1α水平和WOMAC评分、H/Q逐渐升高,伸膝肌群和屈膝肌群PT逐渐降低(P<0.05)。Spearman相关性分析结果显示,KOA患者NLR、OPN、HIF-1α水平分别与WOMAC评分、H/Q呈正相关,与伸膝肌群和屈膝肌群PT呈负相关(P<0.05,P<0.01)。结论KOA患者NLR、OPN、HIF-1α水平升高,并与膝关节功能、肌力具有一定相关性。Objective To analyze the correlation of neutrophil to lymphocyte ratio(NLR),osteopontin(OPN)and hypoxia-inducible factor-1α(HIF-1α)levels with knee function and muscle strength in patients with knee osteoarthritis(KOA).Methods A total of 82 patients with KOA admitted to the hospital between April 2020 and May 2021 were enrolled as KOA group,while other 40 healthy physical examinees were enrolled as the control group.The levels of NLR,OPN and HIF-1α,knee function[Western Ontario and Mcmaster University Osteoarthritis Index(WOMAC)]and muscle strength[peak torque(PT),peak torque ratio of hamstring and quadriceps(H/Q)]between the two groups and among KOA patients with different disease severity were compared.The correlation between NLR,HIF-1α,OPN levels and knee function,muscle strength was analyzed by Spearman correlation analysis.Results The levels of neutrophils,NLR OPN,and HIF-1α,WOMAC scores and H/Q in the KOA group were higher than those in the control group,while PT values of extensor and flexor muscles in KOA group were lower than those in the control group(P<0.01).With the aggravation of KOA,NLR,OPN,HIF-1α,H/Q and WOMAC scores increased,while PT values of extensor and flexor muscles decreased(P<0.05).Spearman correlation analysis results showed that the levels of NLR,OPN and HIF-1αwere positively correlated with WOMAC scores and H/Q,and negatively correlated with PT values of extensor and flexor muscles(P<0.05,P<0.01).Conclusion The levels of NLR,OPN,and HIF-1αincrease in KOA patients,which are correlated with knee function and muscle strength.
