课题达成型品管圈用于人文化出院带药新模式构建效果分析

摘　　要：目的探讨课题达成型品管圈用于人文化出院带药新模式构建中的效果。方法成立品管圈并开展头脑风暴,选定“基于人文化医院出院带药新模式的构建”为本期品管圈活动主题,遵循计划-执行-检查-行动(PDCA)循环,选择课题达成型品管圈开展活动,评价活动实施前(2020年10月至2021年1月)、实施后(2021年2月至7月)有形成果和无形成果。结果有形成果,住院药房审方药师人员配比由实施前的10.00%升至实施后的22.50%,出院带药服务患者满意度由88.24%升至97.48%,出院带药不合理处方率由10.50%降至4.50%,患者取药平均等待时间由5 min降至2 min,出院带药漏取率由6.00%降至0;无形成果,圈员的凝聚力、发现并解决问题能力、沟通力、自信心、拓展思维、荣誉感、责任感、展示自己能力的评分分别由实施前的3.3,4.0,3.6,3.1,3.3,3.3,2.5,3.3分升至实施后的4.5,4.5,4.6,4.0,4.1,4.5,4.0,4.4分。结论课题达成型品管圈活动适用于新模式构建等较复杂的课题,能有效提高住院药师的药学服务质量和自身各项能力。Objective To investigate the effect of the quality control circle(QCC)with project achievement type on the construction of a new humanistic discharge medication mode.Methods The QCC was established and brainstorming was carried out.The theme of the QCC activity was"construction of a new humanistic discharge medication mode",which was carried out through the PDCA cycle.The QCC with project achievement type was selected to carry out the activity,and the tangible and intangible results before(from October 2020 to January 2021)and after(from February to July 2021)the implementation of the activity were evaluated.Results The tangible achievements showed that compared with those before the implementation,the proportion of prescription-checking pharmacists in the inpatient pharmacy increased from 10.00%to 22.50%,the patients’satisfaction with discharged medication service increased from 88.24%to 97.48%,the unreasonable prescription rate of discharged medication decreased from10.50%to 4.50%,the patients’average waiting time for taking medicine decreased from 5 min to 2 min,and the missing rate of taking medicine at discharge decreased from 6.00%to 0.The intangible achievements showed that the QCC members’scores of cohesion,problem-finding and solving ability,communication ability,self-confidence,expanding thinking,sense of honor,sense of responsibility,and ability to show themselves increased from 3.3,4.0,3.6,3.1,3.3,3.3,2.5,3.3 points to 4.5,4.5,4.6,4.0,4.1,4.5,4.0,4.4 points,respectively.Conclusion The QCC with project achievement type is suitable for more complex projects such as the construction of a new model,which can effectively improve the quality of pharmaceutical care and in-patient pharmacists’abilities.
